Question : Hi,
I have a small rash at the top of my buttocks. At times it is a little itchy but not always. I am a 36 year old male otherwise fit and healthy. What could it be? See photo. Thank you

I have gone through your query and it seems to be an eczematous rash.
I would recommend you to apply topical steroid like clobetasol propionate cream on the affected area twice daily and take antihistaminic like tablet cetrizine 10 mg once daily at night for 2 weeks.
Wear cotton underwear, stay in cool airy environment and avoid tight clothes.
