Question : My son is 13 and had a high bilirubin level of 3.1. It was 1.8 in 2018. His alkaline phosphate ? is 454. Is that normal? PCP is sending us to GI Dr. All other labs were normal. Exam was normal. What could be wrong?

Hello and thank you for choosing "Ask a Doctor" service for your query,
Have seen your details and I appreciate your concerns,
Was an ultrasound abdomen done?
By exam you mean which examinations? Physical examination by doctor or Examinations like ultrasound abdomen or CT Abdomen.
Secondly why were these tests done in 2018?
What are the symptoms of child for which examinations were indicated?

Need details for a professional reply.
Waiting for answer.
Thanks!

He was not feeling well for a couple of days. Stomach hurt. Took him to Dr. Labs are current that are attached. They were completed last Tuesday. They just felt his stomach etc... stated everything felt fine. They did do an X-ray that showed he was constpated. No ultrasound or anything else. He feels fine now.we do have a GI appt but feeling worried. Was hoping you might could tell me what might could be wrong.

Detailed Answer:
Hi again,
Bilirubin levels less then 3 are clinically insignificant and bilirubin level of child is 3.1 so though it's elevated but clinically(from Doctors) point of view it's near normal(towards higher side).
His ALT(liver enzyme is also mildly elevated which again have no significance..Liver enzyme level 3 times the upper normal limit is significant).

So as off now very likely either it's nothing or may be a case of mild Hepatitis A(a common problems in children's and its self curable in 1-2 weeks).
Two tests recommend by me are-
1) Hepatitis A
2)Ultra sound abdomen.
If both are fine then no need to worry.
Thank you.
