Question: I am a 61 yo male. 5'10 174lbs Stent in RCA and two stents in left leg femoral artery. Family of heart disease CT shows left carotid blocked 50% I take 5mg Norvasc and 100mg Losartan. My blood pressure (I have charted it for 2 weeks) runs as high as 187/99 to today's 169/97. I am fatigued and have occasional left side chest pains. Currently being treated with the medication listed above with my PCP. Should I seek treatment from a cardiologist and could you even begin to diagnose this steady, documented high blood pressure. I am concerned about having a stroke. I watched my dad have them and it is sad to see.

I would explain as follows:

Detailed Answer:

Hello,

Your blood pressure values are really high.

This means that your current therapy is not very effective in controlling blood pressure values.

For this reason, I would recommend increasing the dose of amlodipine to 10mg daily.

If your blood pressure values are still high after increasing the dose of amlodipine, I would recommend adding Hydrochlorothiazide or torasemide.

It is also important taking antiplatelet agents ( aspirin, clopidogrel) considering the fact that you have stents.

If you have dyslipidemia, statins ( atorvastatin, simvastatin) are necessary too.

If you continue to have chest pain, I recommend going to the ER for a physical exam, a resting ECG and cardiac enzymes, coupled with other blood lab tests ( complete blood count, blood electrolytes, kidney and liver function tests, thyroid hormone levels, etc.).

Hope you will find this answer helpful! I remain at your disposal for any further questions whenever you need!
