Question : I just need to knkw when or if I should go to the ER. I have had diarhea and vomiting for about 24 hrs and extreme lower abdominal pain since last night. Beforethat I had my period and a lot of lower back pain.

Have seen your details.
Before 2020 I would have said that very likely it's viral gastroenteritis and would settle down by itself in a day or two with some over the counter medications like-
Tablet Zofran 4 mg thrice for vomiting.
Tablet Prizolec once a day.
Lots of fluids if possible oral rehydration solution.
Tablet Imodium 4 mg twice daily.
But as we all know vomiting loose stools could be a symptom of COVID 19 also therefore you could go for a rapid antigen test.However if Covid is now not there at you place then you need to visit ER only when pulse is more then 110/minute and blood pressure is less then 100mm( upper).
