Should I Need To Go For Cystoscopic Examination To Rule Out Cancer Of Prostate?

default
Posted on Tue, 3 Oct 2023
Question: Before the invention of the Cystoscopy, how was prostate cancer or benign growths in the prostate or bladder diagnosed?

I saw a Urology PA today who tried to strong-arm me into agreeing to have a Cystoscopy done. This will not happen.

I said, this will not happen. Neither awake or under a full dose of Propafol, a Cystoscopy or any type of foley catherter will not be used on me.

It will not. I hope I am getting through to you that it will not happen under any circumstances.

If I have to put a bullet in my head to make sure of this, I will do so. But a Cystoscopy will not happen.

I was actually not given a stardard prostate exam by my regular doctor, or by the Urology PA. My Internal Medicine doctor stated he could tell by the way I was walking that I was inflammed, so he would rather not torture me with the prostate exam.

I have only had the basic urine tests.

They have ordered a CT Scan with and without contrast, and I am on another three weeks of Cipro. I have been on it one week, 500mg twice a day. Before this, I was on two weeks of Macrobid.

They have not given me a prostate massage either, nor done the four cup test.

They want me to return in two-to-three weeks for the Cystoscopy, but ONCE AGAIN, this will not happen.

I don’t wany anything besides cold facts. Tell me if anything else can be done instead of a Cystoscopy. I have read about a new Virtual Cystoscopy procedure, but I can’t do this either, because it involves a foley catheter.

Please just answer the question.

Brief Answer:
Not mandatory to do Cystoscopy to diagnose or rule out Cancer of prostate.

Detailed Answer:
Hello,
You have been advised to go for Cystoscopic examination to rule out cancer of prostate .At the outset I would state that it is not mandatory to do Cystoscopy to establish diagnosis of cancer of Prostate .

The standard protocol to establish diagnosis is as follow .
1) Urologist will first do basic screening blood test of Serum PSA. If the PSA is more than 4 then only he would go ahead for further investigations .

2) He would then do Digital Rectal Examination to find out whether there is a hard nodule palpable on posterior surface of prostate.

3) If both above criteria are positive then he would go ahead for ultrasound guided biopsy of prostate which is the most confirmative test to confirm the diagnosis the grade of the cancer etc .

4) Ultrasound Scanning of abdomen and pelvis and Trans Rectal Ultrasound scanning (TRUS) .This is routine test and helps to diagnose benign enlargement of prostate (BEP) and Cancer of prostate as well.

5) Uroflowmetry to study urine flow pattern .

6) Urine routine test and Urine culture .to rule out infection .

7) Other specific tests like C.T Scan ,Bone scan or MRI are advised if the prostate biopsy has confirmed the diagnosis of Cancer of Prostate. This is to confirm the stage and grade of cancer .

Thanks and Regards.
