Question : extreme pain below right breast..cramping that feels like a muscle contracting; can't walk. started at 6 pm with forceful vomiting and diarrhea, lower left side hurts too. can't lay down. I have been up all night. increased thirst, chills, weak, can't really move.. I ate shrimp at 4 and then tomato soup. I have pcos and have been spotting and cramping for 1 wk. I also have high bp and take 20 mg of lisiniprol.AGE 43 female. should I go to er now? I have been waiting b/c when I get up I don't have strength to stay standing and fall to floor. then I get dizzy and sweaty.

Thanks for your query.

Noted the history and understood the concerns.

Such a severe pain that is not allowing to walk, lay down or even breath properly needs urgent medical attention.

Hence I would advise you the following in such a situation:
Rush to ER asap for clinical evaluation, examination and investigations as follows:
EKG.
X-ray chest and abdomen.
Ultrasonography of abdomen.
Relevant tests of stool, blood and urine.

All these will lead to a proper diagnosis and gives plan for management of the patient.

Till then, be nil orally.

Take antispasmodic to relieve pain and take a PPI like Omeprazole if you have.

Hi Dr,
It turns out I had a ruptured ectopic pregnancy. I required emergency surgery and removal of a fallopian tube. Thank you for your help. I am back at home and resting now. Best, XXXX
