Brief Answer: you should change the drug Detailed Answer: Hello XXXXXXX You have developed some bad side effects to this molecule at a low dose. Some anxiety is acceptable from venlafaxine initially but your symptoms suggest significant adverse reaction. Had i been treating you I would have gone for a change in drug altogether. This side effect might reduce with time but there will be fear of reccurance with every dose hike. Taking up such tension is neither healthy nor desirable. So, it is best to change it now only. There are many good and safe molecules available in the market- sertraline or escitalopram, etc. You can discuss this with your doctor & get one of these. Or you can decide and let me know and I will help you. You are currently on the lowest dose of venalafaxine and this can be stopped at once and new medicine can be started. wish you good luck Please ask more queries for clarification Dr. Manisha Gopal MD Psychiatry

Thanks for the valuable advice. I would like to add more.I had given a career aptitude test and based on the results, the clinical psychologist stated that I had severe depression and anxiety and suggested me a appointment of a psychiatrist from a well known hospital. I approached the psychiatrist and told him my history. He said i had no such mental illness at all but a anxiety disorder. He said some chemical is released from the brain which needs to be controlled so he advised me to take Venlor 37.5 mg for 5 days and higher doses for further 10 days. On receiving the side effects I again called him up and he said that these symptoms are common. But my mind is not accepting to take the second dose.So I took the first dose on Monday and skipped it on tuesday. Please advise what to do?

Brief Answer: can take sertraline and etizolam Detailed Answer: Hello, It is fine, dont worry, all these side effect should wane away within few days on there own. These side effects also point to the fact that your body is sensitive to develop adverse effects and even a lighter molecule can show good results. You can take sertraline 25mg in morning after meals, and include etirest 0.25mg in morning and in afternoon to help in anxiety fast. One is able to stop the second drug within few days when the reponse to sertraline builds up. If there are sleeping issues- clonazepam 0.5-1mg at night is useful. Donot worry about addiction as you will be tapering it in just few days. You can discuss these with your doctor, he would know about them. Apart from that you can even follow dietary changes( if possible for you). they help significantly. If it is possible for you to pursue these comfortably, i can try to guide you..If not, even then the meds will definitely help you. Wish you good health! please feel free to ask more questions if you have. Dr. Manisha Gopal MD Psychiatry

Thanks for the valuable advice. A brief about my history: I am Nilesh Shirgaonkar from Pune. I am a MBA Marketing post graduate.I did my MBA IN 2003 and since then I am switching jobs because I am not comfortable in high pressure sales target jobs. I usually have fear about direct marketing, approaching people and convincing them face to face.Especially in front of dynamic and dominant people.I feel I am not good in oral communication and I feel most better in written communication.I feel uneasy to make friends fast and people see me as a silent person. I have fear about heights and feel uncomfortable when driving on a flyover.I fear about driving a 4 wheeler though I have a 4 wheeler license. Now a days while driving I feel sometimes whether I have hit somebody but in reality it is not the case.I have a habit of taking up things and leaving them half in between.I have taken quite a number of courses and left them in between. I lack determination. Basically my mother is a silent person and she is not that bold. She doesn't speak much. My psychiatrist says that this is a genetic problem and may be from birth due to mother. So do you feel that Venlor can solve all these problems for three months? along with clinical psychologist counselling.OR your suggested drugs are also helpful? My steyn test score is 3.If you know about aptitude testing,I can scan my test report and send it to you. The test revealed that I am having depression and constant fear about something.Kindly advise and thanks a lot for the earlier advise.

Brief Answer: sertaline with etizolam will help greatly Detailed Answer: Hello, Your symptoms are indicative of anxiety disorder which has been colored by depression later on. I think combination of meds and therapy will be best for your treatment. You should definitely start drugs, they will you a lot Along with meds, get in touch with a therapist and start sessions. They are costly but worth the price when done properly. Sertraline 25mg along with etizolam 0.25-0.5mg will help you work better. It is possible that you might have got the symptoms from your mother, but still this can be managed. You are a different person from her and have a different life style. It is not a 100%genetic disorder. So, dont worry about all this and work on treating yourself. Please ask more questions for clarification. Wish you good health Dr. Manisha Gopal MD psychiatry
