Such a small pin hole below the chest can be due to the following causes:
1) Firstly if it has been present since a long time then it can be a congenital sinus fistula opening in the skin.
2) If it has appeared recently then it can be a gastrointestinal fistula (an abnormal opening that allows the contents of the stomach or intestines to leak and may present as a hole in chest through which sometimes the contents of intestines may leak.
3) Last possibility is of a small sebaceous cyst underneath the skin and the opening may project as a small hole in the skin.
You need to get it examined from a surgeon and get a CT scan of the area and a fistulogram done. Fistulogram involves injecting contrast XXXXXXX into the opening of the skin of an enterocutaneous fistula and then taking x-rays.
