                                    Brief Answer:
Underlying cause needs to be identified
Detailed Answer:
Hello,
I understand your concern. A stiff and painful back may be due to either muscle or ligament strain, or back arthritis.
A strain can happen due to a trauma, even when not aware of it. For example, people who are not in the best physical shape, can risk having muscle sprains even after seemingly light activities, such as lifting or abrupt movement. The sprain leads to muscle spasms which can be really painful.
The good news is, that muscle strains can heal on its own, but you may use over the counter pain killers, such as Tylenol or Advil, as well as ice and heat packs, or acupuncture to decrease the pain and inflammation. Rest and light activity is recommended.
If the back stiffness and pain have been present for longer than a couple of weeks, it is important to have an X ray or MRI done, to rule out arthritis, bone damage, loss of joint cartilage or bone spurs.
I hope this answers your question. If not, please feel free to add more information regarding your condition so I may be able to give you a more informed opinion.
Take care
