Suffering From Esophagitis. No Improvement Seen After Taking Neksium. Should I Continue This Medicine?

Question: Hi ,
I am suffering from esophagitis. The doctors recommended me to take neksium 20 mg once a day in the morning for 2 months . I am on my 3rdweek since I have been taking this medicine , but there is no improvement
Should I continue to use the medicine and wait or should I take a 2nd opinion

Can you describe your symptoms in details, so that I can form some opinion about your diagnosis. If you are suffering from esophagitis, then Neksium is the right medicine, though the dose used should have been more. Even with 20 mg dose there should have been some improvement.

I will suggest you take Tab Neksium 40 mg twice a day before meals. If you feel better in 7 - 10 days then it is fine, otherwise you certainly need a second opinion and may be an endoscopy as well.

In addition you should reduce spicy, sour, fatty meals. Also reduce tea and coffee intake. Stop smoking if you do.

Do tell me your symptoms in detail, I should be able to know if these are due to esophagitis or something else.

HI Doctor,
Thanks for your reply, infact my doctor in AIIMS also suggested me to take neksium 40 mg only once. But the problem is with the discomfort i have with the gas getting formed again and again near my chest and shoulder area. I also have pantop on emergencies before 20 mins of having the meals which gives me relief.
I just want to break this endless loop of gas formation as it is really irritating breaking confidence at times.
I will take your advice to consume neksium 40 mg or should I break it and consume 20 mg before breakfast and 20 mg before dinner.

I think you take 40 mg twice a day to start with. Later as you feel better you can reduce it to once a day in morning.

To start with you can take twice a day for 15 days and then assess how you feel, if you feel better you can shift to once a day dosing. If you keep feeling fine then you continue with one tab per day, if you do not feel well with once a day dose, then you can again take twice a day dose.

Dear Doctor,
I have started the dose of 40 mg neksium from today onwards.I have few questions :
1) What is the expected time of healing time for esophagitis to cure?
2) How long should I take the medicine prescribed?
3) What is the diet which should be taken?
4) My current diet is : Breakfast : Oats , Mid meal :green tea/Orange, Lunch: 4 chapatis and 2 sabzi salad with curd, Mid meal: green tea/Orange or bhunna chana dinner : 4 chapatis 2 sabzi salad and curd
(Salad started from yesterday)
5) Why Numbers of burps are less during the night when I am home?
6) Why Number of burps are more during official hours?

1) What is the expected time of healing time for esophagitis to cure?

Normally esophagitis takes about 12 weeks to heal, but in your case it may not be plain esophagitis. It may be non-ulcer dyspepsia, which may need treatment with drugs like Neksium for a longer period. It will be better, if your symptoms persist, to get an endoscopy so as to know your gut from inside. At that time doctor can also do a H.pylori test. H.pylori if positive will also need separate treatment. Even esophagitis in majority of people is associated with reflux, which also rquires long time treatment with similar drugs otherwise there is a recurrence.

2) How long should I take the medicine prescribed?

Take for about a month then decide according to your response.

3) What is the diet which should be taken?

Your current diet seems ok. In general take less of spices, less of sour food and fried food, less of tea and coffee, avoid choclates. Also avoid anything that you think cause you symptoms.

Generally burps are a symptom of stress. Obviously office hours are more stressful and hence probably the reason for aggravation of burps.

Thanks for the advice. I got my endoscopy done in the month of Jan'13 in AIIMS. Doctors reports say that it is esophagitis.
