Question: Wife was tapering of cymbalta from 60 to 20mg every 3 months sinc last year. 14 days ago suffer servere dpresession. Will not show, eat regularly, and most important will not leave the house to go outside. Says she scared of the outside, showers, people. She regressed back to a little child. She curls up in a ball in bed like a baby and will stay in bed from the time she goes to sleep at 12 mid to 4pm the next day. This is every day. She has learning issues( went to 8 grade only special class, also has no coping skills. Came from alcoholic abusive father as a child till age 18. Alot of PTSD from that. She is now back up to 60 mg cymbalta daytime, 4mg valium daytime, 300mg seroquel, 4mg valium, 5mg ambien bedtime. For 2 wks now. Says shes scared all the time. What can we do?

Consider only two weeks with 60 mg of duloxetine, 4 weeks after starting dose of 60 mg she will be better. In the short term, taking medication and support, give love and affection is the only thing to do. In a few weeks when she's feeling better, it is recommended to be seen by a psychologist and evaluate perform psychotherapy.

Whats causing her to be petrified of going out of the house, shes frozen in fear, will not shower or eat on her own, and have these paranoid thoughts, scared even to go out to see a psychiatrist. One psychiatrist says she has depression with psychosis, another says she is bipolar, and we should up the seroquel to 400mg and possibly add lithium. Another says she has discontinuation syndrome from coming down from the cymbalta too fast, but we came down 10mg every 3 months till we hit 20mg and that when the problems stated. Also shes very anxiety ridden, how do we get rid of this, seems to be a major stumbling block, brain wizing around in fear. Should we stay on 300mg seroquel or up it. This all happened before 1- 2004/2005, overdosed from panic, in hospital, stbilized her, loads of drugs, came home , hide in back in bed, another Dr added lithium, she came back to normal. 2- 2008/2009, overdosed from panic, in hospitalization, loads of drugs, hospital stablized her, came home hid back in bed, another doc added lithium, she came around back to normal. How come these hospitals cannot get it right? She tells me her father used to beat her and tell her you better be sick for you to go to the hospital. also could her limited education and unser development be effecting her recovery/treatment, she is 63yrs old, it been 43 yrs ago, one Doc said you cannot have PTSD from 45 yrs ago.

Excuse me, if she suffered abuse 45 years ago may not have ptsd. Psychotic depression or bipolar disorder are likely diagnoses. The cause of these pathologies is unknown ( can influence genetic, consumption toxic ...) .To treat psychotic depression or bipolar disorder ,prescribed drugs are right and is not necessary to add lithium.
Current dose of quetiapine is enough,if after 3-4 weeks it does not improve, may be considered increasing the dose. I am sure that within 3 weeks with duloxetine 60 mg + 300 mg Quetiapine she will be better.
