Question: I recently relocated and found another psychiatrist as I have been diagnosed with ADD and GAD, as well as a recovered alcoholic. My original pdoc titrated me up to 2X70mg vyvanse tabs, one at 5am and one at noon, which worked great as I found Vyvanse to wear off after 6-7 hours. However, my new pdoc is against 140mg and wants me to cut back to one 70mg tablet in the morning. I tried this one time w/my original pdoc, couldn't handle the irritability, disorganization, etc which always started around noon and had a relapse. Is there any documentation, case studies that I can show my new pdoc that 70*2mg vyvanse tabs is what works best for me? As I realize the Shire case studies that show 70mg to be the XXXXXXX was done on 12 year olds and younger. Any help is greatly appreciated as I see him tomorrow, 4-17-14.
I should also mention I'm 42 years old, 6'2", and 270lbs
Brief Answer:
70 mg is the maximum dose approved by FDA
Detailed Answer:
HI
Thanks for using healthcare magic. I, Dr Abhishek Kapoor, would help you in this query.
Vyvanse contain lisdesamfetamine and its half life is 7 to 8 hours. That is why, your doctor has prescribed Vyvanse 70mg BD. Maximum recomended dosage of Vyvanse approved by FDA is 70mg and above that it could cause serious cardiac or psychological side effects. Even, i would not suggest dosage more than that.
If you are facing problem with once in a day dose, you can try 30 mg BD of Vyvanse or can shift to amphetamine 60 mg BD. In both way, you would able to come out of such psychological side effects like irritability, disorganization etc.
In case, you need further help, you can ask.
