Question: What can
Be done if chemotherpay xeloda and irinotecan has not worked? Bowel cancer operation was march 2014. A further met has appeared while on irinotecan .

Hi

Since a further mets have developed with xeloda and irinotecan in a case of bowel cancer i will now recommend to use the new drug ZALTRAP also called ziv-aflibercept.

Tumors depend on new blood vessels to grow. Without these blood vessels providing nutrients and oxygen, the cancer cells cannot grow. The VEGF (Vascular Endothelial Growth Factor) family contributes to the growth of new blood vessels.

ZALTRAP works by binding to VEGF family members VEGF-A, VEGF-B, and PLGF, which may help to block the growth of new blood vessels. This includes normal blood vessels as well as blood vessels that supply tumors.

Major side effects of this drug are low white blood cell count, diarrhea, too much protein in the urine ,increased blood pressure, decreased appetite, low blood platelet count and increased blood pressure.

You can disuss the drug with your treating physician.

They said he can't have more chemo as his cancer prob resistant to it . Is this true ?

They said we can't do nothing . Isn't their approach wrong ?

The mets are as

Liver met small
1 node small
1 sub pleural nodule small

Can these be ablated ?

Ok thx . We are considering nano knife . Can anything be done to control the micro mets ?

Does 2 failures mean all chemo will not work ?

Will this now effect survival time ?

Yes nano knife is a good option.

To control the micro mets Regorafenib oral tablets can be taken. Discuss the drug with your treating physician.

2 failures does not mean all chemotherapy will not work.

Third line of chemotherapy can be tried.

Average survival time is around 2 years.

If mets are small and few and we do nano knife few times can survival b more even ? 2 years is for those with very extensive mets right ?

Yes as the metastsis is small in size and are few in number if you do ablation survival will be more than 2 years.

After ablation Regorafenib tablets which are oral chemotherapy tablets needs to be taken to prevent the metastasis from coming back.

Hi

Side effects of Regorafenib are fever or chills, rash and headache.

The tablets will not cause hear attack.

It may cause slow or fast heartbeats so you can do 2D echocardiography test to see the condition of the heart before starting the drug.
