                                    Brief Answer:
Infection on phallus
Detailed Answer:
Hi,
Thank you for your query. I can understand your concerns.
Your doctor has prescribed antibiotic and anti-fungal cream.Hence most likely fungal infection with super added bacterial infection is a possibility.
A close-up view of the lesion only is needed to have a possible diagnosis.
Is there any lymph gland enlargement in the inguinal area (groin fold area)?
Is there any extra-marital exposure?
Regards

                                    Brief Answer:
Spots on phallus.
Detailed Answer:
Since you have not uploaded any close-up of lesions, I am unable to arrive at causes. Apparently as its not responding to antibiotic with anti fungal ointment, we should also consider possibility of scabies.
Scabies can cause red, raised spots on the shaft of the penis; but usually it is associated with itching. Standard treatment of scabies is Ivermectin 12 mg PO single dose (available solely by prescription).
Otherwise it may be molluscum contagiosum,a self-limiting viral infection of the skin.
Meanwhile continue antibiotic as prescribed ,which in any case will look after secondary infection, if any.
You can discuss these possibilities with the treating doctor.

                                    Brief Answer:
Genital sores
Detailed Answer:
Photograph of the lesions as a cluster of red spots/genital sores with erythematous base on glans penis is suggestive of genital herpes only caused by the herpes simplex virus (HSV).
Diagnosis can be confirmed by serologic assays that identify antibodies to type-specific surface proteins (epitopes) of the two viral subtypes HSV-1 (gG1)
and HSV-2 ((gG2). T
Acyclovir is the agent most frequently used for the treatment of HSV infections and is available in oral, and topical formulations.
Consult a dermatologist.
