I have gone through your question and understand your concerns.
As discussed previously first step will be to diagnose the cause. This needs examination by a neurologist and investigation like Mri spine, xray chest, blood investigation for myeloma, vitamin D.
The pain killers like etoricoxib or Aceclofenac are effective in most cases.
In nonresponsive cases opioid are better but requires prescription and supervision to avoid abuse. This is also not available over the counter in many countries.
Tramadol can be tried too.
Tapentadol is an opioid pain killer.
