Suggest Diet For Pulmonary Embolism

Question: I suffered from pulmonary embolism about two weeks back. Had difficulty in breathing with syncopal attacks accompanied by cough.every bout of cough was followed by syncopal attacks lasting for few minutes ,pulmonary ct angiography showed bilaterl thrombus with embolism and 2D echo showed lv and la megaly. I was put on iv heparin.Now i am on acitrom2 mg, tab thyronorm 25 mg, pantoprazole20 mg,doxophyline200 mg bd, duolin and budecort nebulisation, prednisolone took 30 mg x 5 days 20 mg x5 days now 10 mg( tapering off ).
Pl advise for how long should i take doxophyline( i have allergic bronchial asthma also),,
Diet exclusions?
Precautions to be taken to avoid this situation
Can i take paracetamol when my headache aches.
Thyronorm with acitrom-any problem?

Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
Definitely pulmonary embolism is a very serious and critical state and it should be managed in a proper way with anti coagulation to maintain INR between 2-3.
Acitrom dose has to be modified according to INR levels.
You can continue Doxofylline initially for 2-3 weeks followed by Maintainence of asthma on inhalers.
Acitrom should be taken at a fixed time daily and should not coincide with the timings Thyronorm.
Now as far as your dietary precautions are concerned particularly you need to avoid the consumption of green leafy vegetables and walnuts.
Yes you can take Paracetamol occasionally for head spin.
Make sure that if you have any medical issues ever don't do any self medication.

Dr XXXXXXX thanx for the reply.i have few quieries--
1) How frequently should i get INR tested? Last week it was 2.3. Should I also get PTT tested along with.Last week it was 33.4.
what is the minimum dose of acitrom?
2)Regarding inhaler, which inhaler should i use, i mean which would be recommended for me? and how many puffs to be taken daily and for how long?
3)I take acitrom at 6.00pm and thyronorm in the morning. is it ok?I am interested to know as to why acitrom is asked to be taken at 6.00pm.
4)Can I take almonds?
5)Can I take curd-/&/0r milk products?
6) Can I take coriander chutney? it is greenleafy.If no, what kind of chutneys can i take?
7)Earlier I used to take Telma-CT and Nebivolol 5m ,both OD since 8 years. When I got admitted 10 days back for PE, since then I did not take it as my BP was normal.Since yesterday, my head is aching and Ichecked mmy BP is 130/70, pulse-82/mt.So, I took these two medicines. My question is - should I now continue taking these anti-hypertensives? Preferred time for taking them?
8)Should I continue with Glycomet? The endocrinologist had asked to take it(500mg-1 od)as my glycosylated Hb- was 6.8. I had taken for only one month prior to my admission 10 days back.Since then not taking. My RBS- ranges b/w 72-130.
9) If PCM se headache ok nahi hua toh can I take pcm+diclo combination?
10)Can I take Calcium,b-complex and evion? I was taking it earlier.Not taking since admision? feel weak.

Hello. Following are your answers :
1. INR needs to be tested every alternate day initially then once a week and then once a month once dose stability of Acitrom is achieved. No need to get apTT checked. Dose of Acitrom varies from person to person based on INR levels.

2. I would recommend Levolin inhlaler that too as or when required basis. Your doctor would examine your chest and decide the appropriate one based on the chest congestion.

3. Acitrom can be taken at 6pm. It should be taken at a fixed time of the day. Evening dose is advised so as to get minimum interference with the food as the food may create problem in appropriate absorption.
Thyronorm has to be taken in morning empty stomach.

4.You should avoid almonds.

5. Yes you can take milk products.

6. You should avoid all chutneys.

7. You should not take anti hypertensives at this BP until and unless directed by your treating Physician.

8. I won't recommend you Glycomet at this time. Your blood sugar is normal.

9. You should just take Paracetamol and not Diclofenac or any other pain killer.

10. You may take calcium, b complex or Evion.
Weakness would go with time.

Few more quiries--
1) today after 7 days I got my INR checked .it was 2.13. PT was 24.6 sec.Mean Normal PT was 11 sec. Is it ok?
2)today i went for follow up - my phsician advised seretide accuhaler 250 mg, montair fx,avamy nasal spray, nebivol 5mg, doxophyline 200mg bd (thyronorm and acitrom 2mg to continue as it is) .
3) Regarding physical exercises- up to whT LEVEL OF EXERTION I SHOULD GO?
4) to improve lung function what breathing exercises should i do? Is spirometry useful? Naturally what XXXXXXX needed?
5)Can curd be mixed with Milk products? some say to avoid.
6) Can I take rice?
7) Bcomplex which one is recommended (as some bcomplexes contain Mg, Se,K etc.)
8) Ca i eat Non veg dishes like chicken ,fish?
9) How long can i continue taking Acitrom?
What are the other precautions to be followed?

Following are your answers:
1. The results are acceptable. Get a repeat INR after 7 days.

2. You may take them as he must have seen your chest condition and advised you accordingly.

3. Start with walk followed by brisk walk till you are comfortable. Gradually develop stamina instead of going for heavy exercises right away.

4. 3 ball spirometry is the most helpful. Do Prayanam and Alom-Vilam. Learn them from a Yoga trainer.

5. Fresh Curd can be taken. Should not be stored and chilled one.

6. Yes you can take 1 cup of rice a day.

7. You can take Becomplex.

8. Take boiled chicken and fish. Avoid as far as possible.

9. Acitrom treatment goes for 6 months or life long depending on the cause of your Pulmonary embolism. Your doctor would decide that.

Just you should not take any medicine withoit your doctor's advice and also important is that you should not go on best rest for long.

Dr XXXXXXX in my case no one could tell/explain the cause of PE. I have no recent history of fracture,no chest pain. Only i had high grade fever about a week before this PE episode.ECG showed only tacycardia. and rhonchi in the chest. yeah, earlier i was on anti-hypertensives as told earlier.My lipid profile is also in the normal range. I want to understand how and why i suffered from this dreadly thing. Pl explain possible causes and how to find them in oneself (for analysis), so that i remain cautious. Regarding family history my paternal side all are on antihypertensives.

1)I do not have diabetes. glycomet was started by the endocrinologist about 50-55 days back coz my glycosalated Hb was 6.8. hE told u r prediabetic. I had gone for with complaint of steadily increasing weight since 2 years.(gained 2o kg in 2 years,had suffered an accident 2 years back wherein i had fracture of my right foot and anterior ligament injury of my left knee.so my exercises were drastically reduced. i used to gym daily for 1 and half our which stopped.After frcture was ok i could not resume gym coz op pain in L knee. MRI of L knee also showed bruise. only limited exercise was there which i think was not sufficient).Moerover that time for the first time my TSH came raised.(my mother and younger sister were also on eltroxine ).So the doctor started glycomet 500mg OD andthyronorm 25mg OD.Gilcomet i took only for one month= i used to feel weak and BS used to go down.At every health chek up my FBS used o be 100 and RBS 110.
Regarding antihypertensives ,i am taking them since 8 years, Earlier was taking atenolol50mg and amlodipine 2.5mg both OD. the dr changed to Nebivilol 5mg and telmisartan-CT both OD since 50 days.
2)I had high grade fever about a week before PE episode.It started with cough nd breathlesness, thought it was bronchial asthma (3 years back i had a bout of asthma. )coz got exposed accidently to dust. Dr advised bronchodialators and antibioticz. took for 1 week . third day fever. widal was positive in low titre. took medicine. fever became ok but cough did not. went on increasing, then had chest pain, discomfort. syncopes and landed up in emergency. During fever i was not static. Used to lie down for some time and get up coz of fear.
3)Pain in calf muscles dont remeber much but sometimes cramps.sometimes cramps used to be very severe. i was advise to take bcomplex and evion.
4) in hospital doppler was done DR told NO DVT.
5)I never took oral contrace[tives all my life till date.
6)medicines-- antihypertensives since 8 years.since 50 days thyronorm. calium (CCM). b compex,..since cough as advised ws/am taking montair fx, doxophyline200 bd, thyronorm 25mg, acitrom 2mg seretide acuhker and nasal spray.
Have started walking-mornig walk and breathing exercises. I get tired very soon.

Hello you have previous risk factors for pulmonary embolism like hypertension and diabetes and your episode got provoked due to fever you had leading to pulmonary embolism.
So, you would need to continue Acitrom probably for life long.
Few doctor's prefer to give it for 6 months and then stop.
My preference is to give it for life long.
You can carry on with all medicines you have been given. You should do moderate brisk walking daily.
As far as weakness is concerned, after such a prolonged hospitalization, this much weakness is apparent.
