Question: Your question: "Sir my hubby was dignosed with fatty liver on XXXXXXX 16 this year.his sgpt was 186 and sgot was 140.never drink or smoke.his ldl cholesterol levels were elevated.he doesn't have diabetes for sure.he started taking evion 400 daily and exercise and diet control also.his sgot got normal yes.his sgpt dropped to 66 but den he stopped taking evion and within 2 months sgpt increased to 91. sir does he need to take evion for life??he was taking 400 units bd.if if he stop taking evion after some months or years den will his sgpt levels increase again despite proper exercise and diet control? ?his wt is 74kg and ht is 5'11".thanks in advance sir.

I am Dr.R.K and I am pleased to assist you.

Your husband is having non alcoholic fatty liver disease.
Some of the common causes of NAFLD are obesity, diabetes, high cholesterol etc.

Your husband's BMI is 22.7. So he is not obese. ( Obesity/overweight - BMI more than 25). He is also not a diabetic.
The risk factor is high cholesterol . He needs to take medications to bring it under control. Only then his fatty liver will become normal. You can get a prescription from his doctor.
Evion is just a supportive medication. He need not take it lifelong.
He will get more benefitted if he takes Trimeg.

Hello sir now his ldl cholesterol and triglycerides have come to normal after taking atorvastatin.sorry i forgot to mention that.but still his sgpt was 91.
Can sgpt be elevated despite normal cholesterol levels also??his hdl cholesterol is 40.
Sir do you think fibrosis has set in and that's why sgpt is elevated??
and sir is it it definitely XXXXXXX or simple fatty liver.

Other causes of raised liver enzymes should be ruled out.

Detailed Answer:
Hi,
Within 3-6 months of eliminating the risk factors, the fatty liver will become normal.

I presume an ultrasound scan was done to diagnose fatty liver. Simple fatty liver will have normal liver enzymes. Since he is having raised enzymes he is having steatohepatitis. Whether it is nonalcoholic steatohepatitis (NASH) or steatohepatitis due to other causes can be known only with more tests.

He should be tested for other causes like Hepatitis B and C infections, autoimmune diseases, metabolic diseases of liver.
You can consult a gastroenterologist/hepatologist for these tests.

To know whether fibrosis has set in a fibroscan may be done.

Your first post was only about fatty liver and I answered all those questions.
In your follow up questions you gave information about more tests which you didnt mention in your first post itself and the list was not complete.

But you wanted to know what more should be done.

That is the reason why I asked you to upload all the reports so I can have a look at it and see what is missing.

But even after that you gave information only about

- Liver function test ( which again is incomplete and not showing the alkaline phosphatase and protein values)

- Lipid profile

- Thyroid function test

- Serum amylase and lipase

- ESR

You can see it yourself that the list is incomplete without the following reports -

1. HbsAg
2. Anti HCV
3. Anti LKM1
4. pANCA
5. Anti SLA/LP
6. Alpha1 AT
7. Serum Ferritin

I wanted to know whether these tests were done. That is the reason I asked you to upload the hepatitis panel and autoimmune work up reports.

The ultrasound report was also not mentioned .

It would be easy for me to analyse if the whole report was given instead of asking about tests one by one.

Since you have not uploaded any of these reports and in your earlier post have mentioned hepatitis and autoimmune were normal, I presume all these tests were done.

If even after 6 months of controlling cholesterol he still has steatohepatitis he needs more tests like -

1. HBV DNA
2. HCV RNA
3. Liver biopsy

The answer -
If the raised enzymes are only due to nonalcoholic fatty liver and he is obese, and all the other risk factors are under control, then exercise and diet control will prevent the disease progression.
But he is not obese. That is the reason I asked you to rule out other causes.
