Question : Hi,
My name is XXXXXXX I am on Lexapro 15 mg, Keppra 1000mg, Klonopin 3 mg and Solian (amisulpride) 100 mg. At low dose (under 300 mg) Solian has dopaminergic activity. To avoid this (because it creates to me some "OCD like" troubles) now my psychiatrist decided to give me a lower dose of Solian, from 100 mg to 50 mg soon. Iwould like to know if i would have LESS dopaminergic activity and not too very much anxiety with this change.
Thanks beforehand XXXXXXX

Detailed Answer:
Hi XXXXXXX and thank you for using my direct link. I consider you my private patient, and will do absolutely everything I can to help you. I hope this can provide continuity to your care on this site.

I've read your question carefully, as well as your history of previous questions, and I believe I have a good picture of your history.

Amisulpride at low doses functions as a dopamine partial agonist, with higher D2 blocking effects at higher doses. It appears you are well-read and already aware of this. At the doses you describe (50-100mg), this drug increases dopaminergic activity, and is used as an "activating" agent for schizophrenia, dysthymia, and/or depression. What your doctor has done is lowered the dose from 100mg to 50mg, which will indeed decrease the amount of dopaminergic activity, and decrease the amount of OCD/anxiety-like activation you are feeling.

Based on the psychopharmacology, I believe with this change you will experience less anxiety, less OCD-like symptoms, and will feel better in this regard.

Do keep me updated with further questions, whether they be about diagnosis, medications, or medication changes. I'd be happy to give you my opinion. I also do therapy electronically if you are interested.

I hope this answer was helpful to you.

Warm regard,

Dr. Sheppe
