Brief Answer:
Decrease dose to 20mg

Detailed Answer:
Hi,
Welcome.
This is Dr Sameer, cardiologist.
With the bp fluctuations you are telling, I would recommend you to decrease dose of Telista to 20mg & try it for some day. The bp could increase temporarily but it will gradually settle down.
In the meanwhile monitor your bp regularly.
I would also suggest you to decrease salt intake & do some exercise.
If you have any more question, I'll be happy to help.
Thanks & take care
