Brief Answer:
Will need steroids.

Detailed Answer:
Dear Madam,

Thank you for posting your query on HCM.

Well looking into your problem I can imagine what you are going through because gouty arthritis attack happens to be the most painful arthritis attacks ever . Since its been 3 days and both Advil and tramadol are not working I will suggest you to talk to your treating doctor regarding giving a short course of steroid in low dose or consider giving an intraarticular cortisone injection which can bring immediate relief . I can understand your pain but don't worry even with no treatment gout subsides within 7-10 days . Any other query please ask . Also you didn't mention the dose of Advil you are taking to look for any scope of increasing it .

Take care.

Dr. Shruti
