I would advise you to take albendazole 400 mg single dose for deworming purpose.
Pantoprazole or ranitidine group of drugs for 4-5 days if you are having acidity problem.
you should take frequent meals 4-5 times a day, fat and dietary product like milk, curd, butter and ghee will help you in gaining weight.
Don't skip any meal, chew it well and take soup or appetiser before large meals.
Multivitamin supplement are also recommended
If you non vegetarian than take fried egg and meat.
You should exercise regularly which will help by improving your appetite and overall well-being.
If it doesn't work than I would advise you for a battery of investigation like CBC, ESR, liver function test, blood sugar and urine microscopy test to find out any underlying cause for low appetite.
