Question: I ask two things. I have wrote you a book about my ears.

I would like to be provided in simple laymens terms what happen to my ears and is there any chance my hearing will come back?

I would also like to get a prescription for the most powerful expectorant on the market that does not cause drowsiness. I have found that if I can keep my nasal passages clear this will also keep my Eustachian tube and middle ear clear which allows me to hear better.

I have gone through your query and would try to help you in the best possible way. First of all i'm sorry to tell that as per rules online prescription can not be given. You need to get prescription from your local doctor. I usually prescribe Guaifenesin to my patients. It has shown very effective results.

Coming to your main query, from your past history i concluded that you are suffering from "Eustachian tube Dysfunction" (ETD). If you have undergone any test(s) then please share reports. They will help me in guiding you better. There are a number of causes for ETD,most common include:

- Nasal congestion from an allergy
- Cold or other upper respiratory infection
- Ear or sinus infections
- Environmental allergies
- Activities with large, rapid altitude changes, such as flying in an airplane or scuba diving
- Presence of obstructing tumors in the nasopharynx

Now to treat ETD we need to first establish the cause. There are certain tests like tympanogram, CT-PNS(para nasal sinus), nasal endoscopy & pure tone audiometry. Above all, physical examination from a consultant ENT specialist. If you can share your area then i can narrow search of some ENT specialist and you can chose from them.

Treatment of ETD includes endoscopic balloon eustachian tuboplasty & Frenzel's Maneuver. Along with procedure you will be given medical therapy as well. But as i mentioned earlier, all treatment options would depend upon the test results. All these treatment options can then be discussed by getting an appointment with a Consultant ENT specialist.


Simple layman term:
To achieve results of getting hearing back to normal you need to get above mentioned tests. They will tell the degree/extent of infection. It will decide the prognosis.

See my file. The above mentioned tests were uploaded some time ago. Can you not see them?

I want to know how just diving 4 feet under water for 3-4 hours could have cause such permanent damage to my ears. There was no trauma, no fever, no pain, not bacteria. Just two days after my dive, I lost 50% of my hearing. Please xplain this to me.

Today, my ears feel like I have my hands over them. So inside and sounds I make come through well and loud. It feels like there is something in my ears like water or mucous.

Please explain this to me.

Thank you for the followup query.

It is unlikely to cause permanent damage when underwater at a depth of 4 feet BUT on the other hand if person has some ongoing pre-existing problem then it may potentiate. Similarly some changes are age related which also decreases the hearing.

Eustachian tube maintains normal pressure in the ear cavity. If for some reasons it is blocked then it causes negative pressure in middle ear cavity causing decreased hearing. Same is your case. Your symptoms are because of not being able to maintain normal pressure. To achieve improved hearing the treatment option are:

- either dilate the tubal opening, by doing balloon tuboplasty, so that it gets wide and doesn't collapse back, or
- second option is insert a grommet (ventilation tube) in the ear drum so that the middle ear is well ventilated to maintain good hearing.

Both of these steps will help to maintain the normal pressure and hopefully your hearing will also improve. You can discuss these with a consultant ENT Specialist.
