Suggest Remedies For Alopecia Areata

Question: Hi Doctor,
I have a small patch of hair loss on my left bottom of scalp. I think it is Alopecia Areata.
I had same symptom few years back and had consulted Dr. XXXXXXX - Dermatologist at XXXXXXX He had prescribed medicine and lotion which I was applying on affected area in the scalp.
I have seen the patch again and would like to have medicine and lotion prescribed. Currently I reside in XXXXXXX and unable to contact Dr XXXXXXX who had earlier given remedy.

Thank you Doctor. Iam attaching pic wherein patch can be seen.
Kindly prescribe suitable lotion and medicine as Iam not interested to pursue for any injection.

I am dr. kakkar (dermatologist). I have gone through your query and I have noted your concern. I would have preferred if you would have uploaded an image of the affected area in "Reports Section" for confirmation of the diagnosis.

Treatment of choice for a focal patch of alopecia areta on scalp is intralesional injection of a depot steroid preparation e.g triamcinolone acetonide 10mg/ml (Tricort or Kenacort injection). The injections may be infiltrated into the bald patch either in 1:1 dilution or without any dilution. Injection may require to be repeated after 3 weeks. Two or 3 sessions of injections are usually effective.

There are other commonly used treatment options for focal alopecia areata but they might take longer to show visible results-

-Topical ultra potent steroid lotion e.g either halobetasol propionate 0.05% lotion (Halox lotion), clobetasol propionate 0.05% lotion (Topsor lotion).
- Minoxidil 5% solution (Mintop forte solution)
- Tincture folica (benzoic acid, Vitamin K and salicylic acid).

Ideally I suggest you to visit any nearby dermatologist and discuss about this option of Tricort 10mg/ml injection or Kenacort 10mg/ml injection (triamcinolone acetonide 10mg/ml) or else you may use Halox 0.05% lotion at day time and Minoxidil 5% solution for night application on the bald patch. In addition, I also suggest you to take an oral biotin supplement e.g H vit forte tablet, once daily for 3-4 weeks.
