Suggest Remedies For Fluctuating Blood Sugar Levels

Question: Thank you doctor for asking for update. I am still experiencing high evening and morning blood sugar. I dropped the Amaryl 2mg because of hypos before lunch. Just the GulvusMet 50/850 twice a day.
Yesterday at 10pm my blood sugar was 10. At 3am it was 8.1 and at 6am 9.7. I don't understand why it remains high for such a long period.
Since March this year I have been on DHEA supplement 25mg once a day because of low energy and low sexual desire. This has improved substantially. Can this be the cause of the difficulty in managing my blood sugar? My doctor doesn't think so but I have read on the net that it could interfere with blood sugar management. My pharmacist has also recommended Kombiglyze 2.5/1000 once in the evening instead of the GalvusMet. What do you think? Is it a safe medication? I read on the web that the DPP-4 inhibitors can cause pancreatitis or even pancreatic cancer. Is this true?

I read carefully your query.

1. I can see that you with Galvus/Met 50/850 twice a day , your blood glucose levels are slightly high, but with an hypocaloric diet and more exercises (if this are not contraindicated for you) you can reach a good diabetic control.

2. Amaryl stimulates the secretion of insuline by your pancreas and for this reason can cause hypoglicemia. In this cases, is a good decision to decrease dosage of the amaryl(If you have stop taking amaryl and you have high blood sugar 2 hr after the meal,1 mg amaryl before breakfast ,in my opinion is necessary for you).

3. Your 3 am and fasting blood glucose is slightly high , so be careful with the meal taken in the evening(decrese the carb intake).

4. DHEA administration has resulted in some degree of insulin resistance and therefore may decrease the effectiveness of oral hypoglycemic agents (drugs used to lower blood sugar levels) and insulin.So, my answer is ,Yes it might be a reason of your actual diabet imbalance.

5. Kombiglyze XR contains a combination of metformin and saxagliptin.(it a combination similar to galvusMet).It is safe,of course but i think that the results in your diabetis control will be the same as GalvusMet (This doesn't mean that you shouldn't take this combination of anti-diabetic drugs).

I know there are a host of anti-diabetic medications around but from your practice which one would you have recommended for me and why.
From your answer on DHEA should I stop taking them and are there alternatives that do not affect blood sugar but improve energy and libido?

It is true that there are a lot of antidiabetic drugs,this is a good think for the diabetic Type 2 pacients.

In your case ,i' m very happy that you want to reach a perfect diabetic control.Your actual blood glucose levels are not significantly high,so you dont have a big work to do.
For you i will insist with the treatment:
1.Amaryl 1 mg fasting

2.Galvus-Met 50/850 mg after lunch and dinner

Don't forget the diabetic hypocaloric diet.(this can help to improve your insulin resistence and to have a better control of your diabetes even with DHEA supplements.

2.DHEA supplements is a good choice for the problem of libido but my advice is to use it cautiously because of their side effects especially if this product is used for long periods of time.
Blood sugar and the treatment may need to be monitored and medication adjustments may be necessary.
You should know that the U.S. Food and Drug Administration does not strictly regulate DHEA herbs and supplements.

Here are some tips that can help you to feel better and improve your sexual performance:

-start by cutting out the parabens and heavy metals in your self-care products,
-stop eating food that you know is bad for you, eat more healthy fats,
-get a good sleep
-stay active, and even increase your activity level over time.
