Question : My daughter is fifteen years old. About three weeks ago, she found a dent on the top of her head. She has not fallen or banged it. She did have a headache for two days around the time she discovered the dent. It doesn't hurt to touch it. I am quite worried as this seems very unusual.

Sudden notice of dent on head in this age which is not painful can be cyst, lipoma or hematoma(collection of blood by unnoticed trivial trauma).
I would like to know if the dent size is increasing, decreasing or same since beginning.
A proper clinical evaluation helps in diagnosis of the condition.
I won't get worried as this kind of lesion is common. Most of the time it is superficial (not connected to brain) and usually does not causes any serious problem.No specific treatment is needed unless the size of lesion increases very rapidly.

Here I would suggest you to get her clinically evaluated by a physician for proper diagnosis and management accordingly.

It is more of an indentation or depression on top of her head. It is not soft to touch. She has had some headaches but not sure if these are related. Thanks for your help.

To me, the indentation in head appears to be just an incidental finding and also I don't find any relation between the indentation and headache.

There is no known entity that may cause indentation and headache.

Hence I would request you to better consult a pediatrician for a direct examination.
