Welcome and thanks for posting your query to the forum.

I have diligently analyzed your query and can make out that you are quite troubled with the redness of the skin and to reduce the scars further.

Scars are the most troublesome problem related to acne. Once formed these are very much difficult to remove completely. I can make out that you have taken most of the latest modalities of treatment including derma-rollers and Fraxel laser and do not want to go further invasive treatments.

In view to help you best I request you to kinldy upload few good resolution pictures of both sides of face and from front so that I can analyze the grade of scars and suggest you best measures accordingly. For that you have a feature on the right side of the page.

Welcome back and thanks for posting your pictures as required.

I can make out you are a smart young fellow with scars over the cheek areas basically. I can further delineate from the pictures that left cheek is more affected than right.

There is still redness and pink areas on face. Let me first reassure you that the scars are not that deep as it was expected. In acne we many times find deep pitted scars which are really difficult to treat and are very much prominent even from a distance.

In your case these are presently shallow sloping scars with red areas. For your further information let tell you that the red areas and the pink spots are sign scar remodelling. It means that new collagen (tissue that makes our skin smooth) is still.forming at good pace and new skin is forming.

In any of the scar treatment procedures we basically orient the treatment towards new collagen formation. That's why you must have had observed redness after ever sitting. In my experience this redness is beneficial as it indicates new collagen formation.

So you should not be worried about the same as is a beneficial process.

As you do not want to go for such kind if procedures which induces redness so in such cases I prefer dermal fillers. It is a simple collagen based injection which lift up the scars immediately. There is no down time and no post procedure effects such as redness or related things. Once done the effect remains for around more than a year.

Hello,

Thanks for your abrupt response.

1) Since i have already undergone intensive treatments so i just wanted to know that should i wait for skin to heal/rejuvenate (as you said its happening ) or i should again start with the procedure you told?

2) I had undergone PRP therapy wherein platelet rich plasma was injected to my face around 3 injections at a space of 15 days.

How is that sort of treatment when compared to dermal fillers?

3) When you say dermal fillers, are they vitamin c serums? if not, then how about injecting vitamin c serums?

4) As i conveyed i am currently orally applying vitamin c serum before sleeping , how it is to continue with the same ?

Welcome back and thanks for your further follow up.

As I guided you that there is still good collagen regeneration based on the redness and the pinkish areas of the face, so in my opinion it is better to postpone any of the invasive procedures including fillers or lasers or even PRP.

Your main concern is the redness at this time. So in such conditions I suggest my patients to halt any of the lotions or serums or related things. Stopping these things will give relaxation to the skin and will aid in reducing the redness.

I suggest my patients that after waiting for around 2- 4 weeks, one can observe the grade of redness and accordingly could restart the Vitamin C serum on alternate day basis. If the redness increases then can halt and if not then continue to apply on alternate days and then make on daily basis after 4 weeks. So an intermittent therapy will help in maintaining the results as well reducing the tendency of redness.

I guided you that fillers are collagen based injection which lift up the scars immediately. So these could be taken any time when a major social function such as marriage or related event is there.


There is no hurry in taking these on immediate basis. I further suggest you to increase your weight thereby meaning bit of facial fat by good nutrition diet. As I can make out from the pictures that you have bit thin face, so increasing bit of weight and fat content of face will help in masking the scars.

Hello Sir,

Thanks for your such a detailed Note.

I have started gyming for past 1 month now and quite regular with it so as put up some weight and build up.

My major concern is the intake of dairy products - milk , curd - so just wanted to know should i avoid intake of these every other day as i read these trigger acne, make skin more oily and may affect eventually skin healing process?

On the home remedies front , is it a good option to apply tomotoes, cucumber on the face ? Currently i am like eating on daily basis but not applying.

Welcome back and thanks for your swift follow up.

This is good that you are trying to put up some weight by gym and exercise. Try to do more of cardio exercises with less sweating rather than vigorous ones with more sweating. It is because more of sweating indices more redness and chances of re-eruption of pimples.

I also suggest you to avoid any type of protein powders or any supplements strictly. It is due to the fact that these could also induce more pimples.

In my own clinical experience I have not found dairy products to increase acne. In western world there is consensus about not using dairy products. But I found no direct correlation with milk or its related products. So in my opinion you can use the dairy products.

I am strictly against using any of home remedies on sensitive face. So in my opinion I always suggest to not to use any home remedies in your kind skin. You can very well eat tomatoes and cucumber but should not apply any of these.

It is variable from person to person and as per the depth of the scars. It could only be decided on close examination only. In my set I prefer to give 1-2 ml of injection to lift up the scars. If required a further 1 ml cold also be used in next 1-2 month of period.

It may vary form 10000 to 26000 according to different types of fillers used. So you can discuss with the treating dermatologist whenever you decide to go for the same.

As I guided you I previous mail that in your kind of cases I prefer to go with 1 to 2 ml in one sitting. If required I further give a touch up with 1 ml more after one month.

There are different variety of fillers available which varies in terms of persistence of results for 1 year to 3 years. I prefer to go with simpler one first and then if required go for advanced one after one to two years.

You can take a further guidance from a treating dermatologist.

As your skin is already oily and the weather is quite humid so I would not suggest any moisturizers in your case. Applying moisturizers could make things more messy and could also induce new pimples due to blockage of oil glands. Oily skin people barely require any moisturizers.

In your kind of cases I prefer to suggest a salicylic acid based face wash ( such as Saslic face wash) which is also particularly helpful in this kind of weather. Be very gently in applying the same and do not try to rub vigorously. It could be used for two times or more as per the condition of oilyness.

And i apply photonban as sunscreen - so is it fine to continue with sunscreens ?

Papulex could be continued over the affected areas where you observe any small eruptions or pimples. Apply only at night time.

For the sunscreen part : kindly do let me know if you are photoban 30 or 60 or ultra or the exact name of the photoban. In your kind of cases I prefer to suggest photoban ultra gel rather than cream. As cream based applications induces more oilyness on face so it is better to apply gel based applications. If it is gel based then could continue the same.

Skin exfoliation is now a days becoming quite popular means of getting glow and for various other purposes. But it is important to get it done on normal skin only.

In your kind of skin you don't require any kind of exfoliation procedure. It is because of the fact that skin exfoliation causes removal of the superficial dead skin. In your kind of skin it is already quite good renewal as seen by the red skin. So if at all you go for this kind of procedure then the skin can get more unsettles and more redness could be perceived.

So overall in my opinion it should not be done.
