Suggest Remedies For Severe Acne While Treating Anxiety

Question: Hello Doc,

My concern is I have PMS acne. I get severe acne 7 to 10 days before my menstrual cycle. I notice white dots on the finger nails in these duration. I have anxiety issues as well. During the menstrual cycle I get severe cramps. For this I take meftal spas.I follow healthy diet which includes at least 4 servings of fruits. I avoid sugar. Even after following the healthy diet I still get acne. I take vitamin C(1000mg) and Vitamin E(200 mg) a day.

Do help me with query. Thanks in advance.

Hello XXXXXXX Thank you for writing to us

I have read your query and I have understood your concern.
Acne around menstural cycle is a common concern in females. Stress also seems to play a role in pre-menstural acne.
Besides tradtional acne therapy, patients with premenstural acne can be started on oral hormonal therapy i.e ethinyl estradiol and cyperterone acetate, monthly pills e.g diane 35 tablets ( a strip of 21 pills). These pills are started on the first day of the menstural cycle and continued for next 21 days and stopped for next 7 days. This constitutes one cycle (21+7=28 days). Then next strip is started from the 8 th day for the next 21 days, followed by 7 days off and so on, for another cycle for a total of 6-12 cycles.
In addition, I would also recommend tablet spironolactone 50-100mg once daily. Spironolactone also helps to control hormonal acne in females, specially those that happen around the time of menses.
Topically you may use a benzoyl peroxide+clindamycin combination gel e.g Clindoxyl gel for active acne.
Oral antibiotics like Azithromycin Or Doxycycline may also be started in case you have inflammatory acne i.e acne which is red, nodular, pustular or painful.
These medicines are prescription medications and I suggest that you visit a dermatologist in your vicinity for the needful
I suggest you to continue with your healthy diet routine.
White spots on nail Or leuchonychia are not related and are probably due to minor trauma to nail matrix and nail bed.
