Question : I have had digestive pain for the last 4 days. It started as a severe pain in the area where the ribs join. This was the first day. It felt like trapped gas. Since then it has moved to my lower stomach with a constant dull pain. My stomach feels as though there is a constant excess of gas. I experience heightened pain when I eat as I try to digest the food. This has resulted in a loss of appetite. Other than the pain I have no general feeling of illness and am not experiencing any other symptoms. I have now started to lose weight. Can you please advise?

Thanks for your query.
To recapitulate: Male/67 - pain in abdomen 4 days - started where the ribs join - as a feeling of trapped gas - moved to lower abdomen with constant dull pain - excess gas feeling - increases on eating something - lost appetite and started loosing weight too - no other symptoms - took the medicines as you have mentioned.

Your history is definitely suggestive of some intestinal infection or there is another focus of infection in the abdomen.

I would advise you the following in such a situation:
Stool tests for 3 consecutive days preferably.
Ultrasonography of the abdomen and CT scan of the abdomen if there is some positive finding.
Tests of blood and urine.
Clinical examination of the abdomen by a Doctor, preferably by a General Surgeon if possible.
Continue the medicines you are already trying and add on/changeover to another if the investigations or your Doctor suggests so.
Also take probiotic
Stop the foods and beverages which you feel are instigating or enhancing your problems.

The present medication I am taking appears to be having a positive effect. I will continue the medication. Thank you for your advice. I am presently in Thailand returning to a more civilzed environment on the 2nd of March when I will ask my local GP to follow up in the way you suggest. Thanks for your advice.
