Question: To orthopedic dr, or neurosurgeon.
My sister is in a lot of pain. She has seen a neurosurgeeon a few times and he will see her again end of March.she hurts spine midback but he thinks the " barnacles" maybe he means spurs at tailbone are causing most of her pain. Would ice or heat be better? Any exercise for pAin relief? Tnx so much. She is 71 on oxycodone.. XXXXXXX Bohne sister, YYYY@YYYY

To Dr. Spaho or joint replacement Orthopedic surgeon re me, XXXXXXX Bohne.
I had my Reverse complete shoulder replacement a month ago on Feb.8, 2017.
I went to PT for help with knee replacement surgury pain flare up ( surgury was 2009). The rt knee and rt side of buttocks hurts a lot trying to sleep sitting up with knees outstretched in bed. Not too comfortable for sleeping either in lazyboy lounge chair, more comfortable with knees rather low in that chair. My knee surgeon does not like a pillow under knees, it makes knees very stiff.
Is there any way to be comfortable to sleep brecause that,s how my body heals from surgery with extra sleep.
I can,t take narcotics. I,m taking one Tlenol 4 x per day. I was better taking 2 Tramadol at a time 3 x per day 50 mg but now PCP!s nurse practitioner wants me to cut back to one pill 50 mg 3 x per day. I was just starting to have less pain on 2 pills at a time, it sure feels TOO early to cut back. It helps shoulder and knee pain.
The surgeon likes a very slow exercise plan for shoulder. I was going to phys. Therapy for knee exercisesbut then he held my new shoulderand back to remind me to stand up straight while i was dooing leg stretches in 4 directions. To me it felt like he was pinching my shoulder but he says no. Then pain has been flaringup severely for 2 days this week since then.
The surgeon was very pleased with 1 st xray and amazed i was so tough on my 2 med plan, no narcotics, tolerating the intermittent pain. Is pain flareup more from Reduced pain meds? Or from PT touching my shoulder. Anysuggestions for now?
I do not tolerate morphine well, psycotic reaction. Thanks very much for helping my sister too, dr. Sparho. Do you recommend heat or ice for shoulder? For knee replacement? For ITBand pain near rt. Knee. Many thanks. XXXXXXX xxxxxx

P.s. I am 69. Spinal fusion T3-L5 1977, with 2 harrington rods. left knee replacement 2007, rt knee replacement 2009.
Reverse total left shoulder replacement new, Feb 8, 2017.

Previous doses of tramadol are acceptable.

Welcome back and thanks for being on follow up.

Since you had reverse shoulder replacement recently, physical therapy in this region should be postponed, slight exercises instead may be of benefit for now.

Increase in pain level may be connected to both, physical therapy and reduced pain meds dose.

Regarding to your other issues with both knees replacement surgery and spine surgery, physical therapy should be continued in this regions.

In my opinion, you should continue for a couple of weeks to one month, until shoulder condition is stabilized, with previous doses of tramadol ( 3x100 mg) and Tylenol, because these are acceptable doses and resulted in significant improvement of pain.
