Question: I am having problems feeling sleepy despite sleeping well at night. I am 67, diabetic and was recently hospitalized for CHF. I am taking Lasix 40mg daily. I also take Lasix 50mg per day. I have been scheduled for a sleep study but this sleepiness is new and disturbing. Any ideas on how to keep my eyes open? My e-mail address is YYYY@YYYY . Thank you.

Regarding your concern, I would like to explain that on your clinical conditions, excessive sleepiness needs further clinical investigations.

Is is related to congestive heart failure (CHF) and its underlying complications (hypotension, blood electrolytes disorder, cerebral blood hypoperfusion, etc)?

Or, is it due to unrelated medical conditions (such as an independent sleep disorder, like sleep apnea, etc.)?

In such case, it is necessary to perform some medical testing:

- closely monitor your blood pressure values (several times daily to rule in/out possible arterial hypotension) and in that way to properly adjust CHF therapy.

- check blood electrolytes level,

- perform arterial blood gas analysis,

- review a recent cardiac ultrasound report (checking the actual cardiac performance).


Also, polysomnography (sleep study) will clarify any possible independent sleep disorders.

Coming to that point, an effective strategy could be followed to avoid the adverse effects of the responsible factor.
