Question: sir i am slow at learning things
my memory is poor.
i have hearing problems inspite of fine ears.
i have ocd symptoms n anti social.
please help me.
i had an accident in head in childhood.

From your history I can say you have Obsessive Compulsive Traits.
This traits may be part of either obsessive personality disorders or symptoms of obsessive compulsive disorder.To confirm exactly your detail history regarding development,schooling and working is required fro you as well as from your parents.

Still I guess from your narration that you have obsessive compulsive disorder as in personality problem person generally do not feel so much discomfort with their habits.

In OCD main problem is excessive, intrusive, unwanted thoughts that make you to do something to relax your self and if you do not do so, you feel lot of discomfort.This causes waste of time and sometime at the cost of necessary activity.

Until schooling you might be able to compensate this time wasting unknowingly. However after entering to engineering because of more work, you realized that you are wasting your time and feeling discomfort.

As you do many routine in strict sequence and if not able to do so, it is very frustrating.This is classical feature of OCD. Normally we all follow daily routine in some pattern but not so in this much strict manner and if sequence is altered, normal person does not feel so much discomfort.

As your mind is continuously thinking something, you may not be able to concentrate in any communication or talking with your friends.That's why whatever spoken by others you may not able to register in memory and you feel that you do not hear properly.

Because of this problem, you might be not able to concentrate fully on your study, that's why your grade is average though you are hardworking.

As you were not aware about your head injury reports,I can not comment what is its relation to your problem.I suggest you should undergo MRI brain scan once.

As you mentioned not able to speak to girls and competitive corporate world, I guess you may have some social anxiety also.

Solution of your problem is, "Consult psychiatrist as soon as possible". As early you start the treatment, better you would be , which can help to improve your performance in career as well as in life.

treatment can be done after confirmation of diagnosis.if personality problems, psychotherapy would be more important than the medicines.

If OCD is there, medicines and psychotherapy would be a great help.Both can be started simultaneously according to choice of yours and your therapist.
In medicines, SSRI group of medicine like fluoxetine can be used in higher doses by gradually increasing in doses and anti anxiety medicines will also help until you become somewhat stable and ready for psychotherapy.
In psychotherapy, Cognitive Behavior Therapy(CBT) is good choice and other therapies can also be used according to your history of problem.

Dear XXXX,
SSRI - fluoxetine, escitalopram, fluvoxamine, Sertraline, paroxetine are really good medicines. In the world each and every medicine has some side effects. Like wise SSRI also have. But believe me more than 90% patients tolerate these medicines very well. They are quite safe and useful in many psychiatric conditions.

I have seen lots of patients of OCD. But only one patient had improvement with CBT only without any medicine.However that patient- 26/male has consulted very early ,just after 2 months of symptoms, that may be the reason.Otherwise in OCD medicine is must.
But still I can say you may be cured without medicines and only with psychotherapy but for that also consultation of psychiatrist is must.

Please do not stigmatize yourself by not going to psychiatrist. Like wise physical problem, psychiatric conditions are proven origin of neurochemical disturbance and that's why medicines are effective.There is nothing wrong to consult psychiatrist, rather it will be a bold step that you accept your problem and asking for help to treat it.It will improve your life. You can go alone for consultation initially.If your psychiatrist feel that need of parent consultation is there, doctor can convince you for that.Believe me your parents would not thing you weak.

MRI has not at all any side effect, not it cause any damage. Do not worry for that.

One hour daily for bathing and preparing a lot for study are also part of OCD. I believe you have OCD and for confirmation personal communication to you is needed by psychiatrist.

So without hesitation take a bold step of consulting psychiatrist and make your life better.

i met with accident in childhood when a scooter hit my head on the back. what type of MRI should I do?
Is CT scan useful in this? And whats the general cost for mri/ct scan?
Regarding SSRI medicines, which medicine should I take and what should be the dosage for that?
Can i take the medicines rightaway?

As it was an old injury, CT scan would not be useful.Plain MRI brain is needed to rule out if any lesion is there. 99% your MRI report will be normal.But you should go for once, so we can go for treatment in right direction.Cost of MRI range from Rs.2500 to 5000 depending upon which type of hospital(government or private) and type of MRI Machine.

If I were your treating doctor, I would start with fluoxetine 20 mg and increases depending upon response of treatment and simultaneous effect of CBT , maximum dose up to 80 mg.
You must consult a psychiatrist to start the medicine, because by chance if any problem, doctor can help you.

You are not bothering me at all.

Your memory disturbance mostly seems to be OCD. After MRI scan you can confirm it, if it comes normal. Considering your history and the way you narrated your problem , I think you do not have any other problem or disease of memory. But that can be confirmed after MRI and detail history and examination.

Normally fluoxetine should be taken after breakfast.

At least one physical examination and detail history needed to start the treatment. I am not saying that I am denying to treat you. Our legal format and also from ethical point of view , you can start medicines after written prescription only.

CBT is a type of counselling which require 6 to 12 sessions 45-60 minutes each.It is difficult to explain about this in detail here. In short it corrects your excessive intrusive thoughts and help you to prevent compulsive acts.

Pleasr do not hesitate to consult psychiatrist in personal. If you can tell me your exact location, I may help you to consult someone.
