Question: Female, 45 yo, IUD in place for several years. For several days, after urinating I notice bright red blood, but no spotting like with a period. Some bloating noted.also, some mild lower back pain

I have gone carefully through your query and understand your concerns. The first thing that I would need to ask you is whether the blood is coming with your urine or whether it is coming from your vagina. If the blood is associated with passage of urine, it could indicate an urinary tract infection and you need to get your urine tested for routine microscopy. If on the other hand the bright red blood seems to come from your vagina, it is not probably of a major concern. It could be possible that your IUD is causing some irritation of the inner lining of the uterus and hence the blood, especially when you are in a perimenopausal age and your own hormone support might be lacking.

It is from the vagina, and I am not spotting like I have in the past with the IUD. Concerned because the blood seems to be related to the vagina and I only notice it when I wipe after urination. in other words, if I wear a panty liner, no visible signs of blood. If I insert a tampon, bright red blood on end (not soaked of course).

Thanks for writing back. Sorry for the delay due to the time differences.

As I mentioned, the bleeding is probably due to hormone deprivation. Although not something critical, you need to visit a gynecologist and get evaluated for the status of menopause. In that case IUD might no longer be necessary but yeah can be started on hormone replacement.
