Question : I am a 56 year old female who, about 3 weeks ago, at an unripe pear to help with mild constipation. The next day I had terrible pain and gassiness. Since that time, I have had a lot of internal gassiness, gas pain, and bloating. My PCP told me to up my Nexium to 80mg a day (I have GERD) but this isn't helping at all. I am very uncomfortable as this has been 3 weeks not. The gas, gas pains and LOUD noised are worse after eating anything with fiber. I usually eat a (lousy) low fiber diet. Very concerned. Thank you.A year ago, I had normal upper GI, colonoscopy and a abdominal CT scan with contrast: normal

I appreciate your concern

80 mg of nexium is the maximum recommended dose for GERD. If you are still having internal gassiness, gas pain, and bloating despite taking esomeprazole 80 mg daily then its a sign of concern. An EKG would be a good option first, to rule out the possibility of any cardiac involvement especially with abdominal / gas pains. If the EKG is normal the pro kinetic medication like metoclopramide along with sucralfate and bismuth subsalicylate can be added to the nexium in consultation with your doctor. Take plenty of water, avoid night time heavy meals and walk briskly at least 20 minutes after each meal. Most likely the symptoms will subside with these measures.
