Question : I am on day 3 of my period. It use to be very long and heavy with cramps and general pms symptoms but I could function fine. this month Ive had an intense heavy flow and its almost done on day 3, but I have had these crazy intense scary emotional feelings. I feel out of control, cannot focus, concentrate on tasks and had lots of crying/sadness/gloom. Ive never felt like this and feel I am sliding into some horrible mental emotional place. I also had some intense feelings of tightness in throat and around my chest, like I cannot breath or am drowning in these awful feelings. Anxious and panicky feeling, though better today than 2 days ago. this is not me at all, I am not a crier and usually feel quite happy, busy and productive. I am 45, fit and have a great family and good life(despite breast cancer in the past, I have completed reconstruction last winter, mastectomy after 2nd dci non-invasive,her2 neg, no chemo or meds) I don't think these are related although I am absent breast tissue that may help regulate some hormones. I really need help, Im worried and want to get myself back. I feel lonely even with my family around and really do not want to be alone even I another room. This is so weird, please help me sort it out. I have used vitex , maca and ashwaganda in the past, what would you recommend?

Brief Answer:
Menopausal symptoms

Detailed Answer:
Dear
Thanks for contacting Ayurveda with your health concern and following is symptomwise approach:

1. Hot flushes, palpitations, chest pain, irritability, weight gain, vaginal soreness are associated with PMS.

2. Since your were treated for CA breast thus do not take any drug which contains Estrogen (Allopathic or Alternative Medicine) unless evidence of hormone dependent tumor is excluded through relevant investigation and examination.

PS. Although the said symptoms could be related with PMS associated anxiety/ depression due to fluctuations in hormone level but in best interest kindly consult with a Gynecologist for evidence of endometrial hyperplasia and endometriosis.

3. Since Education is an important aspect in post menopausal patient thus:

. Self breast examination every month and report if any lump, discharge or skin change
. Yearly P.V. & Pap smear examination
. Report immediately if P.V. bleeding or discharge after menopause.

4.Take Evanova capsule (CHARAK) but not Menosan (Himalaya) until evidence of hormone dependent tumor is ruled out.

5. Intermittent course of Calcium tablets and Alendronate to prevent osteoporosis

6. For irritability: Tablet Mentat (Himalaya) or Tablet Brahmi Vati (Baidyanath): 2 tablets twice daily.

7. XXXXXXX Kalp Forte is an excellent medicine with proven action on the hormonal axis, uterine musculature and ovarian function.

HI My symptoms really do not fit witht he menopause, I don't have many of those, but some of what is considered perimenopuase. I had uterine scraping to rule out uterine cancer and vaginal ultrasound which only showed slightly thickened lining. my breast cancer was dcis, which is stage 0, but it returned again so mastectomy was highly suggested by all docs. these stage 0 cancer cells are not hormone dependent (her2 neg) I have implants so checking for lumps is difficult, as I have some fat necrosis following fat transfer during reconstruction, but all looks good. Im aware of ashwaganda schizandra and have also thought maca and vitex to be good for overall hormonal balance without any estrogen factors. What herb is good for stress/anxiety ,sad feeling I have been having? They are lessening as this cycle winds down, it only lasted 3 days but was super intense. Im seeking my XXXXXXX which was yanked away from me, it very frighteneing. I eat a clean diet and feel better without dairy, wheat, oats etc. I do yoga , lift weights and workout 5-6. I ride and train horses , run my own business and keep up with the demands of an active family. Im not the average 45 yr old, im aware of my body and have been trying to get it balanced over the past 5 years.
thanks XXXXXXX

Brief Answer:
The Perimenopause Transition

Detailed Answer:
Dear
Thanks for reverting back.

1. As you go through PMS period many of symptoms are result of decreasing estrogen. I understand you haven't reached menopause yet, but this is the time when estrogen levels fall and people start to experience strange symptoms. If the changes interfere with your well being and day to day life consider visiting a Gynecologist for hormonal profile.

Besides the herbs you are aware of off, I will also suggest you take mint, coriander, tomato, cumin, plenty of water, vitamins and mineral supplements, apple, banana, grapes and dates. Avoid: excess physical –mental exertion, sun exposure, corrylium, smoking, errhine therapy, betel leaves, gruel, soup, day time sleeping, garlic, ginger, onion, spicy-heavy- junk food, mustard oil, pickles.

There are also prescription drugs as smritisagar ras, trifla churan, brihat vata chintamani ras, ghritkumari, ashwagandha, bahmi, jatamansi, stresscom which are helpful. If you ever able to meet an ayurveda physician, he / she can let you know about these drugs.

In addition, I will also recommend:
1. You can practice: bhujangasana, halasana, paschimottasana, sarvangasana, shalabhasana, shavasana and vakrasana and pranayamas like bhastrika and kapalbhati.
2. Lemon juice mixed with sugar in a glass of water helps release stress removing hormones from the body and bring mind at peace/rest.
3. Take 1 spoon black pepper powder and 5 spoons jaggery in 4 cup water and boil till 2 cups remain, filter it and drink for 1 month.
4. Mix 4-5 dates in half glass of water and drink 2 times daily for a month.
5. Apple, almond, banana, broccoli, cantaloupe, melon, carrot, cashew nut, corn, cauliflower, grapes, guava, lemon, XXXXXXX orange, pineapple, soybean, spinach have anti oxidant and detoxifying properties so take them in plenty.
