Brief Answer:
Thyroid

Detailed Answer:
I see you are making considerable efforts to ensure sustained weight loss.

Sometimes there can be thyroid problems that make weight loss difficult despite your best efforts. So it would be important to make sure you do not have hypothyroidism ie an under active thyroid.
There is a simple blood test for this called TSH. You do not have to fast for this and it can be done at anytime of the day or night.

Regarding dietary program I see this one has worked for you. But I personally do not recommend one diet plan over another. A simple balanced diet combined with intensive exercise has been shown to be effective.

WIth regards to the other symptoms you have it would be important to have routine tests like
CBC
CMP (liver and kidney function tests, electrolytes, calcium, glucose)

done in order to screen for any other potential conditions that may cause these symptoms.
Note: For further follow-up, discuss your blood glucose reports with our diabetologist. Click here.
