Suggest Remedy For Fluctuating Body Temperature In A Person With Typhoid

Question: MY wife 50 years old tested positive for typhoid fever (Widal test). Took ceftriaxone plus erythromycin combination for 07 days. Fever subsided after 02 days but on 06th day evening temperature was 99.2 and thereafter it ranges around 100 till last 05 days (evening only). Morning temperature is around 97 and noon 98.6 and night temperature again comes to normal. Today she had a bit of melena. Please suggest..

Welcome to health care magic,

As your wife is already been diagnosed with typhoid fever (with positive widal test) then she may need to take antibiotics (ceftriaxone plus erythromycin) for minimum 10 to 14 days and she should repeat the widal test after 5 days of commencing treatment.

I generally give my patients tab.levofloxacin 750 mg and tab.cefixime 400 mg for 10 days initially with other symptomatic drugs and strict liquid diet.

Initially only liquid diet for 3 to 5 days then if response is good, then you can go for soft diet till the completion of treatment as diet is also a important factor during this treatment.

Here,she has taken treatment for 7 days which is incomplete and also her fever is keep coming back in a day especially during evening (99 to 100 fever) time which is a classical symptom of typhoid fever.

Means she still needs to complete the antibiotic treatment.

Also,melena (black stool) is not a good sign which might be indicative of some bleeding in lower gastrointestinal tract (in large bowels).

She may require some routine blood works,ultrasound abdomen and colonoscopy.

So,in my advise you should see a physician who will know about all the necessary tests and can complete the treatment course and if required may change the antibiotics as per patient's compliance.
