Good evening. I am an adult neurologist and would like to comment on your problem.

Inability to focus could indicate a number of different things....INCLUDING FATIGUE though you may not recognize it as such.

I would recommend being seen first by your primary care doctor to do a standard blood and urine workup to look for obvious problems in electrolyte, blood cell counts, and liver/kidney functions. If all that checks out then, I would see a neurologist who will check you out and do a good neurological examination. He may decide after speaking and testing you to send you either to a sleep specialist for testing for things such as sleep apnea and narcolepsy to a neuropsychologist who can detail your ability to concentrate and be attentive.

Monotony behind the wheel can be a sign of sleep apnea or narcolepsy

Lab wise I would make sure I had the following tests run:

1. FT4, TSH (thyroid hormones)
2. Vitamin B12, Folate, Vitamin D (important nutritional elements necessary for good concentration and ability to focus especially when involved in monotonous activities such as driving)
3. serum cortisol levels (tests the adrenal glands)
4. serum Testosterone
5. ESR, CRP (inflammatory markers)
