Question : This all started in XXXXXXX 2013 whereby I was having pain on and off and started losing weight. In April 2013 my body condition started to deteriorate by tremendous body weight lost. Appetite okay. Jun started to have stomachache describing to be cramping pain.

OGDS was done and mild ulcer was found on the opening of stomach sphincter in Jun 2013. Medicines were given. No relieve, in fact pain became worse. Went again twice and they said to do a CT Scan. And there goes terminal ileum was partially blocked (Stricture). Dr.Kunalan said must do surgery immediately. Of course surgery means a big taboo. So went to a second stop Dr. E. Peter(Surgeon) in Pantai. He said do a colonoscope and get a tissue biopsy. But he refered me to gastroenterologist in SJMC Dr. CK Ranjeev.

Done and unfortunately tissue did not suggest much of the three diagnosis (Crohn's, TB, Lymphoma). So Dr. CK Ranjeev suggested to do laprotomy and get a tissue biopsy. I was reluctant to do because it's a major surgery and the surgery was indeed an exploratory one. I asked why not Laproscopic by just few holes and get the tissue instead of opening me fully. Unfortunately SJMC refused laproscopic tissue biopsy worrying fistula formation.

So my office boss arranged with Klang GH head surgeon Dato Farouk to see my case. Few options were tried before the final appointment that I was suppose to go for Laproscopic surgery because no improvement. Finally pain and Barium Follow Through test which was instructed by Dato caused bleeding. End up admission in SJMC in August 2013.

Then Dr. XXXXXXX Arumugam did CT Scan again and said got two strictures and suggestive of Crohn's. And finally Crohn's treatment came in. Pain reduced because heavy load of steroid and Azathioprine given. Managed by Dr CK Ranjeev. Took all these till beginning of XXXXXXX 2014.

So finally wife suggested to go to UMMC as my condition didn't improve. Saw Prof XXXXXXX in UMSC. He gave Infliximab infusion as the strongest medicine for Crohn's which is like chemo but this medicine shuts down immune system. That's when from two strictures became seven. Got two infusion XXXXXXX end and Feb middle. March went to see Prof XXXXXXX again and told him pain unbearable. Started Pethidine jab for pain management. Something like morphine. So powerful. I began to get the jab so frequently till in March I told Prof XXXXXXX not improved with the infusion and he told me get admitted and we put my digestive tract full rest - no food but infusion of food in blood straight for one week. One more colonoscope done and pain improved because I didn't eat.

Then got discharged. That night itself (3am next day) I collapsed at home. My friend who works in the clinic (Thamo) helped me and dad to bring to Arunamari. Took Pethidine jab and pain never stop and I can't get up. From there ambulance was arranged to bring to UMMC.

Here goes my intestine ruptured and about 100 ml pus made my right side ureter closed and right kidney swollen (hydronephrosis) and BP was 60/40. Very bad till I was rushed to emergency laprotomy. 12/3/14 first op. Major repair work done because intestine ruptured and tangled like a ball. Was in the ward and pain didn't come down. Another emergency laprotomy done on the 19/3/14. This time 3 ft of my small intestine involving terminal ileum, jejunum upwards. And a stoma was created.

TB diagnosed on the 2/4/14 and I got discharged on the 3/4/14. My issue is with this short bowel, anything I eat wrongly it gets me into severe dehydration. Not sure how I can put on weight. My uric acid level is three fold of normal range and my Hb is 7.2 and after 2 tablets 3 tds of iron tablet Hb is now 11.8

My concern is when i end up having loose output, i had to pop in antidiarrheoal like sweets just to stop. It aint food poisoning but just irritation to the GUT lining and end up loose output. End up dehydrated.

I am sorry to know the condition that you are in.

I presume that your condition will improve once anti-tubercular treatment takes its effect. Tuberculosis can cause weight loss.

Alongwith you have inflammation of the gut and recurrent diarhea. The diarhea is due to the inflammation itself. Anti-inflammatory drugs like sulfasalazine come to my mind and can be tried. These are more commonly prescribed in inflammatory bowel disease and various types of preparations are available that can be inserted and act locally.

Racecadrotil is another anti-secretory drug that can be tried but is an opioid. Octreotide is another anti-secretory agent. Clonidine is used in refractory cases.

Apart from this you have to improve your immunity and dont let germs take over your gut. For this I strongly suggest that you take hygienic food and drink cleanest water from reverse osmosis technique. Use yoga and breathing techniques to flush out toxins as it is commonly believed that most toxins flush out from breath.
