Question: I have had frequent pain in the right lower abdomen for 8 years. Recent CT scan found a septated cystic structure anteromedial to small bowel on the left measuring 5.3cm. Pain appears to be related to gas trapped in large bowel and the gas is difficult to expel. Worse when lying down especially on the right side. Usually wake up 2hours after falling asleep and then have difficulties going back to sleep. Feel better being up walking and excercising(yoga and pilates). I am 73 years old on no medications besides Omega-3, Vit.D3 and eye vitamins. Tired in XXXXXXX

Brief Answer:
Do regular exercise and continue with FODMAP

Detailed Answer:
Hello. Thanks for putting your query in HCM. I can understand your concerns

Presence of cystic lesion on the left side does not appear to be related to the pain on the right side.

If you were my patient I would have treated you in following manner:
1. Continue with FODMAP diet
2.Do regular walking or yoga
3. For pain take mild antispasmodic agent like combination of clidinium and chlordiazepoxide which is very useful in patients with excessive gas and it also relieves mild anxiety if it is there. It is to be taken thrice a day half hr before meals. It is a prescription drug and your doctor might be knowing about this
4. take a course of probiotic
5. Take capsule simethicone thrice a day which prevents gas formation and absorb previuos gas present in intestine. It is available OTC

I hope I have answered your query and this will help you. Wish you a good health
