Question : 28 Male

Received Protected (not sure about the quality of the comdom) Oral sex from a sex worker (she used mouthwash before). But had some sores on genitial and under testices during that time.

But After 2 months after the incident having slight nausea and headache. Itchy penis tip and Anus.

But Before the incident itself, i had fungal infections near genitial area. And i have diagonsed Gasterities and oesophagitis for past 3 years.

I had master checkup not aimed at hiv (2 months after the exposure) it said slightly elivated billurubin 1.4 (max 1.2), and total protein 8.3 ( XXXXXXX 8.2). CBC normal except little bit elevated RBC and platelletcount.

During the master checkup the doctor suggested that i am having more stress and hypochondriac.

Still For peace of mind, After 3 months and a week I received Hiv 1/2 Elisa negative. (Before taking Elisa test used some anti fungal medications the day before. Does it affect test result?)

Now after 3.5 months having a small discomfort under my ear(Swollen lymph node i guess.), Itchy penis tip. (took medication and other fungal infections are gone).

Kindly suggest whether i need to take another test at 6 months are these symptoms are stress or anxiety related.

Brief Answer:
Your results are reliable. You are free from HIV infection

Detailed Answer:
Hi,
Welcome to HCM
Thanks for posting your query.
Oral sex carries a low risk for HIV. That too, with a protected oral sex there lies almost no risk for HIV infection. Non-reactive HIV tests 3 months after the episode totally ruled out the possibility of HIV infection. Antifungal drugs have no influence on the HIV test result.
You had fungal infection. That could be a cause for your symptoms. Lymphadenopathy can occur to anybody. They are not specif for HIV.
Nothing to worry. Your doctor will cure your problems.

Hi Doctor,

Thanks for the answer.

1.The worker licked my nipple before i asked to stop(2-3 seconds). Does it carry any small possibility of risk.

2. Regarding itchy penis tip, i could not find any abnormality on the tip except darkness between frenulum and the bottom half of the urine hole. Could you suggest tips to avoid this itchyness or any cream.

I used levocetirizine and terbinafine tablets to fight off the other yeast infections (around genitial and anus). Shall i continue the same.? or jus regular cleaning during bathing is enough since its there for quite sometime.

Brief Answer:
Licking of nipple has no risk for HIV.

Detailed Answer:
Hi,
Welcome back.
1.Licking the nipple carries no risk for HIV.
2. There might not be anything over your penis to be treated. This feeling of itching over the tip of penis could possibly due to your subjective feeling of fear or aversion to STD/HIV.
Repeated washing will clear if there is any minor problem. External application would not be necessary. If you have any doubt, you can consult a dermatovenereologist. If you are able to observe any cahnge over your penis you can upload the picture of the same, so that I may able to give more information.

Thanks!
Hi Doctor, Adding to the above follow up images for your verification, I have one final question.

The test result for Elisa HIV 1/2 had mentioned like the below

Observed: Negative | Biological Reference: Negative

Doesnt it contain any value? And what are the chances of error made by the lab?

Thanks!

You are free from HIV.Color change over penis is normal.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
The dark discoloration around the urinary meatus is a normal color change could occur during the course of years. It is normal and have not indicated any pathology. Just ignore the same.

Your nonreactive HIV ELISA test 3 months after the risk contact affirms that you are free from HIV 1 and 2 infection.
This test is reliable up to 98%. Nothing to worry.

Thanks for clearing my queries and desterssing me.
Regarding wet+itchy penis I consulted dermatologist as per ur advice.
But I have few general questions reg safe sex.
1. Would you still consider protected oral sex is less risk still if penis and tesiticels had fungus/sores.
2. There are no documted case of HIV for insertive oral. Is it true? Have you ever came across one in your experience?
3. Many doctors are convinced that insertive oral sex is almost equal to safe sex regardless of sores, cuts in either end. Since you have rich expertise in the field what's ur stand on this?
4. Cold weather can cause stiff lympnodes in ear / neck? If so how long it can be neglected or when to start worrying?

Brief Answer:
Oral sex caries low risk for HIV.Protected oral sex is safe very much

Detailed Answer:
Hi,
Welcome back.
1. Oral sex has lower risk than vaginal and anal sex. Penis would be covered during safe sex, so the fungal infection and other sores of penis would unlikely be transmiited to the partner during protected oral sex whereas scrotal sores and STD lesions would not have been protected against transmission risk.
2. I had come across a case of HIV transmitted through oral sex. But this is very very rare.
3. Insertive oral sex can not be on par with that of safe sex with regards to HIV/STD transmission.
4. Cervical or submental Lymph node enlagement are secondary to sore throat, ear infection or scalp infection or oral diseases. Cold weather can predispose throat or ear infections.
