Question: My 7 yr old son's foreskin was forcefully opened by the doc yday evening and since then he is unable to urinate due to pain. What medicine should I give to ease the inflammation and pain? Thanks

You should give paracetamol (available in brands like crocin or calpol) to your child. The dose is 15 mg/kg per dose to be given at minimum interval of 3-4 hours. The strength of the syrup is either 120 mg/5 ml or 240 mg/5 ml depending on whether it is plain or double strength. So you would need to give the dose as per the body weight of your child.

Thanks Doc! Since the foreskin was forcefully opened by the doc as it was very tight and opening minimally, what should we do after the pain subsides as we could see some white deposits on the glans after it was opened? It was not cleaned by the doc. Should we open again after few days and clean it with soap water as he compains of itching there. He has given cetzine for 3 days and soframycin to be applied on glans. We wish that his skin retracts easily and it is clean. Any oil or lubricant?

Thanks for writing back. Sorry for not being able to respond earlier.

Even though your doctor had forcefully retracted the prepuce, he had perhaps done a good thing in doing so. The condition that you have reported is called balanoprosthatitis. If the prepuce or foreskin cannot be retracted spontaneously, it needs surgical excision, else infection and inflammation can occur. You should definitely open it in a few days and see if you can make the retraction smooth. The whitish substance you see underneath is called smegma. It needs to be cleaned on regular basis. If it is let to accumulate, it can become carcinogenic. Hence regular retraction of the foreskin and cleaning the whitish substance with soap and water is essential. If that can be done, its fine. If not, probably you have to opt for circumcision of your child.

It does not matter if you use plain water, soap or something else to clean it. I think using water and soap is enough. Cetirizine will help in reducing the itchiness while soframycin will help in reducing local inflammation.
