Question: I’m 28 yrs (Height 161 cm and weight 70kg) married women trying to conceive from last 1 year. After 4 failed IUI (3 normal IUI and 1 with 4 gonadotropin injections - last was done in March 2015) I changed my gynecologist who asks me to do hormone tests and semen analysis/survival of my husband. As per our reports my husband’s reports are good with severe infections (pus cells) and in my reports I’ve some hormonal imbalance (she told me PCOS in very small scale). Then she prescribed me Krimson 35, Metital and Fol-XT tablets for 1 month (1 tablet at noon-started taking on 9th day). I’ve completed my course on 5 July 2015 but didn’t get normal periods (just a slight bleeding (In past I’ve hemorrhagic cyst rupture in year 2012 and delay in period for 1 month in year 2011 but after that i.e.. from year 2012 I never had irregular period problem.)
My question is
1.     I’m confused whether I’m taking right medicines or not?
2.     Is pus cell decrease pregnancy chances?
3.     If I’ve PCOS problem then how can I’ve normal periods..?
4.     Why I’m not getting normal periods even after completing Krimson 35 course?
5.     What do you suggest?

Hi,

I understand your concerns about infertility and PCOS.

Following is my reply:

1. Krimson is contraceptive pill. You need not take this when trying to conceive.

2. Pus cells reduce pregnancy chances and need to be treated.

3. You have PCOS and also obesity. You need to reduce weight significantly to have regular periods.

4. Your periods might start little late after completing Krimson 35. Hence please wait.

5. Start weight loss seriously to have regular periods.

Hello Doctor,
Thanks for quick reply.
Till date my periods are regular. One month delay in year 2011. As per my hormonal study in last month my doc prescribed me Krimson 35 for 1 month as report shows imbalance. Yesterday I met her and share period spotting problem she told me everything is normal and now I’m taking gonadotropin injections from yesterday (4 dose x 150) and them ovulation study from coming friday for my 5th IUI. If not succeed then will have final 6th IUI and move to IVF. (I’ll share my hormone report tomorrow.) Now my confusion is....
1.     Though I’m having regular periods and normal ovulation with proper follicle size (in each month + for all last IUI) why I’m facing these problems..?
2.     Is my diagnosis is correct..?
3.     Should I go for 5th/6th IUI OR loss weight and try for natural pregnancy OR directly go for IVF..?

Following is my reply:

1. There is an entity called unexplained infertility where conception is difficult though everything is normal. About 30 percent couple face unexplained infertility.

2. Your diagnosis is right.

3. You can try another cycle of IUI with gonadotropin injections. If not successful, you can opt for IVF next cycle.

I've uploaded my hormones report and my husbands semen analysis report (done on 7th June). Just wanted to know is everything is in control and what are the pregnancy chances with all these reports..?

Your hormonal reports seem to be good.

Your husband's semen has infection. He needs semen culture and sensitivity test done to identify organism causing infection and the antibiotics to treat the same.

So should I go for IUI/IVF now or wait till his infection and my hormone problem is cure?

otherwise we are just taking treatment and there is no success.

Get IUI done this cycle and keep his treatment also on.

I said your hormones are normal and there is no need of cure.

IUI is better option than trying naturally.

If you dont succeed with 3 cycles of IUI, try IVF.

As of now no need of IVF.
