Brief Answer:
you have to ruled out from Buerger's disease

Detailed Answer:
welcome to health care magic,

With your history of smoking, the episodes of swelling are concerning. You should be evaluated to rule out Buerger's disease.

Long standing BUERGERS disease will cause inflammation and some times gangrene. If it is blocking a nerve you will notice numbness in the area below the swelling that is below the nerve. I will suggest that you see a surgeon at the earliest who will evaluate you on these lines. You may also need a lipid profile and electrocardigram.

In the meanwhile, I recommend you to quit smoking immediately. Avoid Alcohol if any. I will also start you on lipid lowering drugs.

Hope this explains in detail
