Question : Hi Dr XXXXXXX

Due to my frequent travel, couldn't post you.

As you suggested, I have limited my excise to walk, slow running, sometimes small distance cycling.
Thank you for your valuable suggestions.

I am feeling far better now but I can't say it's 100%. Few days back I travelled to my native and when returned back the pain started again.
Must be because of change in food and water. I had strong pain in stomach when I push near belly bottom with fingers, the way doctors check. The pain extends till oesophagus. But after a week, the pain reduced and pain near oesophagus also reduced to great extent but little bit I sense even now. Hoping that will go away.

Brief Answer:
pantocid Dsr for 6 weeks followed by plain pantocid for another 6 weeks .

Detailed Answer:
Hello XXXXXXX ,
Good to hear from you and happy that there is benefit in your symptoms.
Complete healing is a slow process and usually takes 3 to 6 months.
I would like to help you understand things better . God has given each of us a tight one way valve at lower end of food pipe , it would allow only food to pass from food pipe to stomach , but will never allow any food or stomach acid to reflux back from stomach to esophagus.
Considering you had low grade esophagitis, ( because you are health concious we could detect it early ) it means that your lower end valve is not that tight as it should be , it's congenital and not that big an issue in most of the ppl .

Now why did I explain you this ?
1) to explain why pressure on stomach increases pain , because it will push the stomach acid from stomach to food pipe ( esophagus )
2) medication will help resolve whatever inflammation has occurred but will not prevent new inflammation from happening.
It means you should take care LIFE LONG .
It's not as scary as it sounds .
It's just you have to keep spicy food , chocolate, high fatty food , tea coffee aerated drinks to minimum .
In your case prevention is always better than cure.

Generally is all our native though the food is not spicy , but our grandparents tend to use ghee and oil , pure fat decrease the tone of valve increasing reflux , don't worry about the same .
In such case take smaller and frequent meals .

Treatment :
For complete healing of mucosa take pantocid Dsr once a day for 6 weeks and than plain pantocid / I prefer a neksium (40mg) for another 6 weeks ( a total 3 months) .
Continue with food habit changes life long ( trust me it's not difficult )
If even after 8 weeks there is occasional burning pain , Urea breath test for H pylori , repeating invasive endoscopy is not justified .
Another thing is barium swallow , which rules out hiatus hernia , a common association with reflux gastritis.
These are just test to re in force our diagnosis and treatment remains particularly same .

Let me know if any clarification needed .
Regards Dr Priyank Mody
