Question: I have pain under my right shoulder when swallowing. Should I be concerned?

Hi! Welcome to health care magic!

Based on the history you have shared, it is very uncommon to have any such relation but still keeping in view your history of breast cancer and graves, two possibilities which come in the mind and needs to be ruled out first are, to do workup for axillary metastasis of any remnants of breast cancer with extensions into neck compressing on nerves supplying the pharyngeal muscles. Another possibility is of cervical rib, which can compress upon nerve roots with the movement of pharngeal muscles, alongwith movement of cervical musculature.

Even retrosternal extension of goitre due to grave disease can produce pressure symptoms which may lead to pressure symptoms of cervical nerve roots supplying the shoulder or axilla.

A MRI of the neck and thorax with axilla may b rule out metastasis as well as muscular anomalies or pressure findings on the structures around.
