Suggest Remedy For Persistent Post Nasal Drip And Granular Pharyngitis

Question: Since last two years I am suffering from Respiratory problem. During winter Sinus pain with Post nasal drip. During Summer Granular pharyngitis and Dry cough. During spring runny nose, itching inside mouth, itching at eyes, watery eyes.
I did chest X-ray no significant founding except prominent vessels. PFT test found OK. I have allergy test Total IgE 1200.2 and allergic to Pollen and dust. I have moderate DNS.
Right now I am taking Fluticasone nasal spray (2 times a day), Airtec capsule inhaler (2 times a day). Montelucast tablet 1 Time a day).
Can u evaluate the situation?

I have gone through the history thoroughly and understood your problem; I will try my best to help you with this.

1. You seem to be suffering from allergic rhinosinusitis. High levels of IgE indicates constant allergic process in the body.

2. You would require a CT scan of the paranasal sinuses to assess the status of the sinuses and blockage if any.

3. Consult a good allergy & immunology specialist to undergo skin prick test to identify the cause of allergy and depending upon the allergen identified, you can get immunotherapy done.

4. Although you are taking the right set of medicines, you would require antihelminthic such as albendazole for deworming and a short course of oral steroids to bring down the allergy levels in the body.

5. Finally, allergy doesn't have cure; prevention is the best treatment. Avoid those things which induces allergy and you will feel better.

I already did Allergy test. Attaching the report herewith for your review. I consulted with Allergiest and Pulmenologist. He sent a request to XXXXXXX WHO to prepare Allergy vaccination. Which will take 1.5 months to arrive after that I will start taking it. I decided to go for Septoplasty next.

Do you think it will ease out things? Right now I am in Central Asia with hard winter. Facing trouble with Frontal Sinus pain. I can not go for Septoplasty immediately. I don't want to take pain killer, can I take steam to reduce pain?

Thanks for writing back

I have gone through the reports thoroughly and my opinion is as below.

1. Your allergy is getting triggered by dust, fungus and mites which are present in plenty in our environment, particularly inside the house. If you are using air conditioner and carpet, it can worsen your problems, hence avoid it.

2. Steam inhalation will given you excellent relief. You should perform at least 2-3 times a day for 3 minutes.

3. Drink plenty of warm water. Use saline nasal spray at least 4-5 times in a day to keep the nasal mucosa moist and for easy clearance of the secretions. You can also try taking mucolytics such as ambroxol or bromhexine to relieve the thick secretions in the sinuses.

4. Septoplasty is required if at all there is gross deviation of the nasal septum which is causing the nasal and sinus block.

My other suggestions are:

1. Drink plenty of warm water
2. Avoid coffee/fizzy drinks/chilled food and beverages
3. Avoid using room fresheners, carpets, perfumes or incense sticks at home
4. Do not sleep directly in front of the air conditioner or under the fan
5. Change your pillow cover every night before going to bed
6. Whenever you are exposed to dust or smoke cover your nose with a clean handkerchief
7. Get the filters of the AC cleaned at least once in a week

Just my final question. I never had this big allergic problem before 2 years. Before may be suffered from sneezing in dusty environment. It all started all of a sudden. Can it happen like this?

Yes, many of the individuals who were perfectly normal for years can get affected by allergy at any point of time in their life. The reason could be the reduced threshold in he sensitivity for certain allergens.

"Prominent pulmonary vessels" are usually noticed in the hilar regions of the lungs. This is the area through which blood vessels and nerves, etc., enter and leave the service of the lungs.

Prominent vessels in the hilar region could mean any number of things or could mean absolutely nothing. Hence, it is considered normal unless there is any other lesions seen associated with it.
