Question: uncircumcised penis hard to pull foreskin back seem swollen a few scars about healed now ?

You have given a very short history. So I might ask a few questions like-
Since when are you having this difficulty?
Do you have local redness and swelling? Any fever?
Any recent injury to penis?
How did you get scars?

Kindly answer the above so that it becomes easier for me to understand.
Anyways, difficulty to retract foreskin is called phimosis. This is commonly seen in children and ideally requires circumcision. In aged people, this is mostly from post traumatic adhesions, diabetes etc. You have mentioned scars, so there might be history of trauma. Also, the swelling you have mentioned can be due to local inflammationof the penis or balanitis.
As management, keep genitals clean; you can use mometasone or other steroid ointments locally which should be helpful. You can take oral antibiotics after consulting with doctor.
You should consult a general surgeon for further management options.
