Suggest Remedy For Rash From Sweating And Irritation On Scrotum, Thighs And Perineum

Question: Hello I'm 29 years old. I play basketball 4-5 times a week for 1-2 hours. I also work in the kitchen where I do a lot of walking and sweating. For the last few days I have had what I thought was just a rash from sweating. The bottom of my scrotum, inner thighs , and perineum have been very irritated. I use the gold bond spray on a regular basis. This time the spray and powder seem to make it worse.... and now there seem to be small red bumps everywhere. Some of the bumps have just began to have whitehead but too small to pop. Very dry and very painful to walk. Have tried triple anti-biotic ointment. Not sure if it works because it hasn't cleared up yet and still bothering me.

The rash may be intertrigo. This rash tends to occur in the groin area, under the arms or under the breasts in women.

These are areas prone to moisture and friction as the skin folds rub against each other.

It can become infected by fungi and/or bacteria.

Treatment involves reducing moisture, friction and treating any infection that may be present.

(1)Skin folds should be kept separated with cotton or linen material

(2)You should also consider the use of a barrier cream. This would reduce friction/rubbing and aid healing

(3)a topical steroid used for a few days (3 to 5) would help reduce the inflammation and pain. Mild ones can be obtained over the counter eg hydrocortisone but if this is not strong enough then you may need a prescription for a stronger one from your doctor

(4)an oral anti inflammatory pain killer would reduce the pain eg ibuprofen, naproxen

(5)topical anti fungal - as mentioned , intertrigo can be complicated by fungal or bacterial infections
Topical anti fungals are ketoconazole, canesten, econazole, miconazole

There are some creams that may combine barrier creams , steroids and anti microbials eg triple paste cream and XXXXXXX goo

Summary: (1)separate folds
(2)topical steroids, anti fungals, continue topical antibiotic
(3)oral anti inflammatory drug

Intertrigo occurs in males or females. It occurs in any area of skin folds- groin and thigh area, axilla and breast area (in women).

The triple antibiotic can still be used. Intertrigo can be complicated by bacterial or fungal infection so if there is a bacterial component the triple antibiotic would help.
