                                            Question : I just returned from a fly fishing trip in Mexico and have a bright red rash on buttocks and back of thigh. We did some wading and also were caught in some rain at times so the nylon wading shorts were wet for hours at a time. Sitting in the boat riding from spot to spot in wet shorts also may have caused friction. The rash itched when it first appeared the day after we got wet but does not itch very much now. It is however, very bright red in color. Stopped at pharmacy on way home from work today and explained my plight to the pharmacist who thinks it may be a condition surfers get called board rash so he recommended Aquaphor made by Eucerin. Wondering if it is an allergic reaction to something in the salt water as it is so bright red and kind of scaly and should see a dermatologist.Your thoughts?

I am a dermatologist and I see a lot of such cases In most patients, it is a combination of friction, moisture, and irritation in varying proportions. They all do well by following the undermentioned measures:
1 Keep the part dry and wear loose and cotton garments only.
2 Use a combination cream with Clobetasone and miconazole at least twice a day
3 Tab Levocetirizine 5 mg per day.
The rash will start improving in a day or so if taken care of in this manner and after that switch over to Miconazole preparation for few more days.

Thank you for your quick response. Can this combination cream of clobetasone and miconazole be purchased over the counter in the US? I have Lamisil cream in my home now which is an antifungal as well but it has a different active ingredient. Please advise.

An antifungal cream can be mixed with a hydrocortisone cream which in all probability should be available as an OTC product) to get the desired results. The two are to be mixed freshly in equal proportions and then applied in thin layer avoiding massage. Please use the mixture thrice a day initially as the ingredients are of milder potency.
