Brief Answer:
Avoid having sex until you get better.

Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

It seems to be local infection of the glans (Balanitis). There are a lot of types of balanitis and yours looks like to be a serious one.

Kindly spare some time to upload pictures of the glans by retracting the foreskin. I feel sorry for the painful situation you have. I will be able to better assist you after seeing and diagnosing the case.

It is also necessary to have your sexual history to get a better idea of the case. Are you sexually active and have had unprotected sexual intercourse recently??

No--You can't have sex right now until you get better. Also avoid scented soaps and any wear loose clothes and cotton only underwear.

Use sitz bath (take a tub full of Lukewarm water and add salt in it and sit in that for 4-5 times a day).

For pain you may take Ibuprofen and if you ever start to feel itch in the course of disease you may take Claritin and rest of the medications can be decided on seeing and diagnosing through the pictures. Try to have good resolution and light for the pictures.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. Please don't forget to give Five Star rating to help me assist my patients in a better way; and, please don't forget to 'Close This Discussion'.
For any disease you have, a timely follow-up is very necessary regarding the treatment efficacy and to see if there is a need to change the treatment options. Brief Answer:
Not actually the shrunken foreskin, but it is inflammation.

Detailed Answer:
Welcome back and thanks for putting up the follow up query.

It is not actually the foreskin shrinkage but it actually is the inflammation and swelling of the glans penis that is causing the retraction of the skin difficulty. I feel sorry for the pain feel for retraction and I appreciate the efforts in uploading the pictures.

Medical Treatment:-
Apply the following ointments on the foreskin for improvement. It will help you a lot.
Tacrolimus ointment.
Hydrozole ointment.
Triple antibiotic ointment.

If you feel pain, you need to take Ibuprofen and if you feel itching take Claritin by mouth. Take plenty of water and after voiding urine clean the area properly with water. Rest of the instructions are same as aforementioned.
